The Intake Coordinator for Emergency Intermediate Housing Program reports directly to the Director of Social Services. The Program Coordinator manages the assigned program to deliver comprehensive clinical-based case management services to homeless families, veterans, women, youth, and individuals with special needs, in a therapeutic community environment. Intermediate Housing includes various program models such as bridge housing, low-demand, service-intensive, and clinical treatment focused on specific target populations per program design. The primary function of these programs is to assist the chronically unhoused in the transition to permanent Housing and case management services to develop a comprehensive plan for ongoing support.
Responsibilities:
  • Facilitates services to assist clients in transitioning to permanent or long-term Housing.
  • The Director of Social Services supervises case management (CM). Provides support and direction to other program staff in the delivery of services, which include but are not limited to case management, counseling, therapeutic services, employment assistance, legal advocacy, benefits assistance, financial management, mental and physical health support, addiction treatment, housing referrals, resident council, transportation, /client appointments, room inspections/general weekly cleaning of client rooms; inclusive of completing and monitoring clinical progress notes and service plans.
  • Reviews client case files for quality assurance according to file audit protocols, including but not limited to ensuring files have proper eligibility and intake documentation, are completed timely upon discharge with required discharge summaries and forms submitted to funders on time, and are audited at least quarterly while active.
  • Creates CM schedule and ensures CM schedule covers a late night; ensures coverage of CM responsibilities as needed either directly or through designated staff
  • Ensures assigned program(s) are operating and performance goals met following company, accreditation, and funder standards. Tracks and submits program data and reports to the leadership team, including developing and following through on corrective action plans.
  • Develops and maintains collaborative relationships with the Veterans Administration, the local Continuum of Care, and other partners to successfully deliver services to veterans in the program
  • Oversees database management and compliance with the Homeless Information Management System and internal electronic case records system to ensure data quality, compile demographics, track services, generate reports, and measure program progress.
  • Employs evidence-based models to ensure quality care, including trauma-informed care, motivational interviewing, and cognitive behavioral therapy.
  • Provides clinical case management to veterans based on program needs which may include having an assigned caseload of clients of any applicable U.S.VETS program as needed
  • Facilitates a Therapeutic Community environment that fosters camaraderie, peer support, responsibility, a sense of belonging, housing retention, community integration, and quality of life for unhoused individuals.
  • Ensures collaboration with veterans to complete benchmarks (goals and objectives) and an Individual Housing and Employment Plan to plan for the unhoused individual goals and transition.
  • Collaborates with Service Coordinator (SC) to provide clinical guidance.
  • Leads discussions on how to approach particular situations regarding client progress
  • Oversee program intake process, assessing unhoused referred to the program in consultation with the Director of Social Services to include any problems, needs, or barriers identified in the following areas: Employment, Housing, Financial, Sobriety, Legal, Family/Social, and Medical/Psychiatric.
  • Responsible for training Case Managers in all aspects of CM responsibilities
  • Responsible for reporting client information to and seeking guidance from the Director of Social Services on various aspects of the program
  • Initiates appropriate clinical interventions when veterans present with behaviors and face disciplinary issues that may affect Housing; this includes reviewing and tracking all program incident reports.
Facilitates regular meetings with program staff and is responsible for generating weekly, monthly, and quarterly reports on program progress and outcomes to executive leadership.
  • Facilitates and attends roll calls and town halls
  • Participates in the purchase process by identifying program needs and reviewing program staff requests for purchases; provides purchase requests to Operations Department for final approval and any guidance for completing the purchase process, disseminating, and tracking inventory.
  • Assists with payroll administration by reviewing timesheets for accuracy, providing applicable approvals, and ensuring timely submission
  • On call and responsible for ensuring an appropriate level of staff are on call 24 hours a day/7, days a week, to respond to program needs.
  • Monitors the collection of veteran program rent/fees submissions and strategizes with the team to assist clients in meeting financial responsibilities and prioritizing Housing.
  • Participates in the budget and billing process in collaboration with Operations & Finance departments and takes action to ensure the program's financial health.
  • Participates in weekly managers' meetings facilitated by executive leadership.
  • Facilitates and/or coordinates individual and group activities/classes by staff or outside providers according to U.S.VETS, accreditation, and funder standards.
  • Responsible for handling assigned petty cash for program expenses, compiling required documentation, and submitting timely expense reports.
  • Performs other duties as assigned
  • Creates and maintains program class schedule/calendar and turns it into the Director of Social Services for approval
Qualifications Requirements:
  • Minimum of two years of supervisory experience in a related field. Two years of employment may substitute for supervisory experience.
  • Experience working with people experiencing homelessness and the veteran population preferred.
  • Bachelor's Degree or Medical Experienced social services field or other related field required. Masters preferred.
  • Excellent written and oral communication skills.
  • A minimum of two years of case management experience is required.
  • Ability to work effectively with a diverse group of clients, staff, and community members.
  • Ability to manage a team in a residential social service setting and accomplish both programmatic and administrative deliverables.
  • Ability to take direction, work independently with minimal oversight, and work within a team.
  • Computer proficient in Microsoft Office and Internet. EHR, EMR Case Management
  • Leadership and conflict management skills.
  • Demonstration of personal and financial integrity in the workplace.
  • Coordinators assigned to the Women Program (ADVANCE or other) must have at least two years of experience working with unhoused/homeless and at-risk female veterans and demonstrate success in applying evidenced-based approaches and clinical interventions in working with this population.
  • Professional manner and appearance with the ability to conduct presentations to clients, staff, and the community.
  • Software and technological experience.
  • A valid driver's license is required. Must meet company insurance requirements.
