Basic Function/Overall Responsibilities: The Incident Management Specialist is responsible for the intake of client incident and staff allegation reports that occur throughout the agency and will collaborate with program staff members to provide status updates and corrective action plans. The Incident Management Specialist also communicates and disseminates this information to appropriate oversight agencies to ensure our organization is in compliance with regulatory requirements. The Incident Management Specialist will review, analyze, and report incident trends to improve the agency’s overall quality and efficiency.


Principle Assigned Responsibilities


  • Review all Incident Reports for completion, accuracy and classification of events; immediate notification of problem areas to programs/supervisors to ensure client safety and appropriate follow-up.
  • Provides technical assistance for the Incident Module located in the Electronic Health Record (EHR).
  • Supervises the agency’s Internal Investigator.
  • Provide employee training to include Mandated Reporter, Incident Management, Investigation training and/or other identified trainings in areas of expertise.
  • Collaborates with program leadership to create, monitor, and discontinue agency staff safety plans in the EHR's Incident Module and Client Record during neglect/abuse investigations.
  • Permanent member of the agency Incident Management Committee (IMC); maintains meeting schedule, records minutes of Incident Management Committee Meetings, chairs committee and manages committee correspondence/follow-up with agency staff.
  • Review monthly statistics to allow active participation in IMC discussion regarding patterns, trends and preventive strategies.
  • Assist in coordination of special investigation assignments of trained agency staff; monitor the status of special investigations and coordinate with the Incident Management Committee to arrange special committee meetings as necessary.
  • Collect and analyze data from Incident/Allegation Reports to identify areas of potential risk.
  • Monitors program response to incidents relative to compliance with agency policy and procedure and regulatory / accreditation requirements.
  • Coordinate agency interaction with the Justice Center, Office of Children & Family Services, NY State Education and OMH relative to abuse investigations, including arrangements for interviews with HGS children and staff, information/document gathering and dissemination.
  • Uploads and monitors investigation information within state oversight agency databases (i.e., NIMRS, WISR, ARTS).
  • Special projects as assigned by supervisor.


Education And Experience Requirements


  • Bachelor’s degree in Human Services or other related field.
  • Three years’ experience in a child welfare, behavioral health or other closely related field. Experience in the area of quality improvement, compliance or involvement in special investigations highly preferred.
  • Supervisory experience, preferred.
