Job Summary:

HealthCare Synergy, Inc. is searching for a Licensed Therapist – Physical, Occupational and Speech – to join its Clinical Team. You will be working directly with Healthcare Consultants and Clinical Advisors specializing in Home Health and Hospice Care settings and provide Telehealth visits; For example: Initial intake and evaluation visits via telecommunication platforms and/or devices. (I.e., Zoom).

Primary Duties and Responsibilities:

The successful candidate will:

• Work closely with Clinical and Health Care advisors.
• Learn and become certified users on Synergy EMR platform.
• Perform visits to home health and hospice via telecommunication (and in-person visits depending on location).
• Meet quality assurance requirements and other key performance metrics.
• Work closely with Therapy assistance.

Job Requirements

• Ability to communicate clearly via telephone and e-mail required.
• Graduated and certified from an accredited School on specified clinical field applying for.
• Proficient in speaking and writing in English. Bilingual is a plus.
• Must possess excellent customer service skills and experience (phone and face to face).
• Able to conduct assessments via telehealth, carry a compact license or have license that is recognized in the following states: Texas, California, Nevada, Arizona.
• Excellent Communication skills.
• Proficient knowledge of Telecommunication device and platforms (and not limited to): Apple and Android devices, Zoom communication, facetime, google meeting, etc.
• Prioritizing, Time Management, and Organizational skills.
• At least one year of experience in a healthcare setting. Home Health, Hospice and Outpatient knowledge experience are a plus.
