General Description:
The incumbent is an integral part of the Iowa City Home and Community Based Services program (HCBS) case management team. This individual must be familiar with services offered within the local VA facility and in the community for home and community-based services including but not limited to skilled home care, non-skilled home care, adult day health care and respite care.
Position specific duties as follows:
(1) Uses knowledge of the VISN 23 Home and Community Based Services program (HCBS) and VA Program to organize delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning.
(2) Applies advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating Veteran needs and outcomes.
(3) Coordinates care for Veterans receiving both internal VA and community based home and community based services.
(4) Evaluates, systematically, current practice and formulates outcomes for groups of patients and/or organizational processes within area of expertise.
(5) Works with inter-professional team members at VISN and facility levels to ensure services and eligibility determinations are delivered objectively with strong, consistent reliability with documentation and communication of outcomes in a direct and coordinated manner.
(6) Conducts comprehensive clinical review of patient records to identify the presence of Veteran medical necessity for services
(7) Evaluates and documents Veteran progress and changing needs.
(8) Executes position responsibilities that demonstrates leadership, experience, and creative approaches to management of complex patient care.
Clinical:
A. Ability to make autonomous clinical decisions in a Direct Access environment. This includes the ability to independently evaluate and treat patients who seek physical therapy services.
B. Performs complex physical therapy evaluation and treatment for male and female patients aged 18-85+ years old. Assessment of the patient's learning needs is performed, and patient education is tailored to meet those needs involving the patient, family and significant others in the process as appropriate.
C. Involves the patient, family/significant other and medical team in treatment planning and goal setting through ongoing communication.
D. Plans and implements a comprehensive treatment regimen designed to maximize functional potential of the patient, using a myriad of treatment modalities and procedures.
E. Demonstrates independent problem-solving skills that allow for frequent modification of treatment plans in response to subtle, but significant changes in the condition of the patient. The employment of innovative treatment techniques is used to obtain the best outcome.
F. Assesses for and requests prosthetic/orthotic equipment as needed and is able to make modifications as necessary.
G. Makes recommendations and referral to other medical specialties/services as needs are identified during ongoing assessment of this complex patient population.
H. Maintains a flexibility of schedule to allow for unscheduled patients, patient/team consultation and mentoring.
I. Maintains consistent and timely documentation of patient encounters in the electronic medical record in accordance with accepted PT practice, facility and Rehabilitation Service policies and procedures.
J. Shares knowledge and expertise about the scope of physical therapy practice and services provided across VISN 23. This is provided through direct consultation, mentorship, development of in-services and clinical training and leading by example.
K. Completes educational activities and competencies to maintain state licensure and any additional certification(s) as required by the certifying/licensing agency and the VA.
L. Completes annual mandatory reviews and other education as directed.
M. Aware of and adheres to the department/service/facility's values, mission, rules, policies and procedures and the American Physical Therapy Association Standards of Practice and Standards of Ethics to ensure quality patient care.
N. Aware of safety concerns, regulations, policies and procedures, and employs appropriate safety
O. Incumbent is required to complete and maintain competency in the Prevention and Management of Disruptive behavior Level 2.

Qualifications

Basic Requirements:
  • Doctorate degree in physical therapy.
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Loss of Credential. A PT who fails to maintain the required license must be removed from the occupation, which may result in termination of employment. At the discretion of the appointing official, an employee may be reassigned to another occupation, if qualified, and if a placement opportunity exists.
  • Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist.
  • NOTE: Individuals must be a graduate of a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited college or university. The CAPTE is the only accreditation agency recognized by the United States Department of Education (USDE) and the Council for Higher Education Accreditation (CHEA) to accredit entry-level physical therapy programs. Verification of accredited programs may be obtained from the American Physical Therapy Association (APTA) at www.apta.org.
  • English Language Proficiency. Individuals appointed under authority of 38 U.S.C. chapters 73 or 74, to serve in a direct patient-care capacity in VHA must be proficient in written and spoken English.
  • Education and experience. The individual must meet at least one of the requirements below:
    • Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist.
    • Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a physical therapist.
    • Doctorate degree in physical therapy.
  • Licensure. Individuals hold a full, current, and unrestricted license to practice physical therapy in a State, Territory or Commonwealth of the United States, or in the District of Columbia.
    • Loss of Credential. A PT who fails to maintain the required license must be removed from the occupation, which may result in termination of employment. At the discretion of the appointing official, an employee may be reassigned to another occupation, if qualified, and if a placement opportunity exists.
  • Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a physical therapist.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: GS-13
Experience. In addition to meeting the basic requirements, one year of progressively complex experience equivalent to the GS-12 grade level and one of the following:
  • An additional advanced degree in a related field.
  • Board Certification in the specialty area recognized by the ABPTS; or
  • Evidence of a minimum of 2,000 hours of clinical practice directly in the specialty area. This time must be between the past two and five years; or
  • Completion of a fellowship program; or
In addition, the candidate must demonstrate the all following KSAs.
Demonstrated Knowledge, Skills, and Abilities
  • Ability to teach and mentor medical center staff in a specialized content area of physical therapy.
  • Advanced knowledge and expert skill to perform and interpret specialized procedures and tests in evaluating the treatment outcomes in the assigned specialty content area (see Definitions, paragraph 2. i.)
  • Advanced knowledge and expert skill in a range of specialized interventions and treatment modalities used in a specialized treatment area of physical therapy.
  • Ability to guide the work of a multi-disciplinary team in a specialized content area of physical therapy.
  • Ability to serve as a consultant and subject matter expert for health care providers regarding the delivery of care within a specialized content area of physical therapy (see paragraph 2i).
