Home Office: Tampa

The Home Health, RN manages a caseload :

• Ensures that all visits scheduled and provided are authorized and covered by physician orders
• Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant, including anticipated weekend visits
• In addition to hands-on care, the Case Manager oversees the activities of the multi-disciplinary team that provides care to the patient
• Informs the scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor
• Monitoring the provision of therapeutic services through written and verbal communication with all disciplines

Home Health Case Manager, RN specific skills include:

• Educator skills
• Interpersonal skills
• Computer skills appropriate to position
• Knowledge of regulatory standards appropriate to position
• Written and verbal communication skills
• Critical thinking and delegation skills

Certifications and Licensures:

• Required RN (Registered Nurse) License
• Driver's License, State of Florida required within 30 days of hire
• Required BLS (Basic Life Support)

Education:

• Preferred Bachelor's Nursing
• Required Associate's or Diploma Nursing

Experience:

• Required 1 year Registered Nurse or 1 year Licensed Practical Nurse
• Preferred 1 year Home Care

Location: HomeCare Tampa

Status: Part Time, Exempt: No

Shift Hours: 8:00 AM - 5:00 PM

Shift: Shift 1

Shift 1 = Days, 2 = Evenings, 3 = Nights, 4 = Varies

Weekend Work: Occasional

On Call: No
