Analyze patient information for completeness, accuracy and authentication.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Under the general direction of the Director, Health Information Management and Privacy Officer and the direct supervision of the Health Information Manager and within established policies and procedures, the incumbent performs the following functions:
1. Retrieves discharged patient information for the appropriate patient care units.
2. Makes new folders when needed and assigns encounters to the appropriate volume in the system.
3. Enters the given names for newborns into the computer system.
4. Maintains knowledge of medical record chart order. Sorts and organizes patient information in the discharged record into the correct chart order.
5. Utilizes computer systems and applications to retrieve missing patient information and include it in the patient record.
6. Reviews discharge records and electronic health records for completeness and authentication within guidelines of the Medical Staff and accepted documentation practices, assign deficiencies in the system as necessary.
7. “Punch” and attach neatly the paper encounters into the chart.
8. Knowledge of Joint Commission standards regarding medical record documentation requirements.
9. Update record location when records moved or sent to another area.
10. Performs accurate Quality Assurance audits, special projects or monitors as requested by Health Information Manager.
11. Participates in appropriate role in disasters, fire drills, fire and other emergency situations.
12. Exercises care in the operations and use of equipment and reference materials. Performs routine cleaning and preventative maintenance to ensure continued functioning of equipment. Maintains work area in a clean and organized manner.
13. Performs similar or related duties as required or directed.
WORK SCHEDULE:
- Generally day shift, Monday through Friday, 8:00AM – 4:30PM.
Education:
- High school diploma or equivalent.
Experience:
- One to two years’ experience in an office setting preferred and knowledge of Medical Records.
