The Health Coach is responsible for improving Mass General Brigham Health Plan members' knowledge, skills, tools, and self-efficacy so they may achieve their self-identified health goals. The primary topics of coaching include addressing social determinants of health (SDoH), healthy eating, weight management, physical activity, stress management, and disease prevention. The Health Coach also assists with Gaps in Care closure by providing member-focused outreach and education.
The Health Coach develops a wellness coaching relationship with the member. Responsibilities include conducting member outreach, needs assessment, and goal setting, and assisting the member through the behavior change process.
This position interfaces with Mass General Brigham Health Plan medical care management programs and behavioral health care management by referring to and collaborating with these programs to support an integrated model of care management.
- Assists members in identifying possible barriers to change and developing problem-solving strategies to overcome barriers to change.
- Coach members around identified gaps in care (such as diabetes management screenings and medication adherence).
- Assist members in creating health and wellness plans utilizing SMART goal planning to meet their wellness goals, and monitor goal attainment.
- Utilizes reflective listening skills to develop a full understanding of members’ health and wellness goals.
- Encourages adoption of habits (behavior modification) that are conducive to a healthy quality of life.
- Facilitates conversation to guide the member towards behavior change using motivational interviewing techniques and ensures the member takes a proactive role in changing behaviors.
- Assists members in creating goal plans that fit within their personal goals and values.
- Enhance member confidence in the ability to make healthy behavior changes and meet health goals.
Education and Referral
- Communicates with care management team regarding member needs and facilitates referrals to the appropriate program(s) when needed.
- Assist with calls to community/ social agencies and health providers when needs are beyond the scope of Health Coaching practice.
- Educates member regarding relevant benefits and services for which the member may be eligible including SNAP benefits, housing, utilities, and transportation services.
Outreach and Assessment
- Interact with members via phone, email, and text as needed.
- Proficiently conduct outreach including use of effective outreach functions (i.e., use of language interpreter services, research of member information, verification of member information, etc.).
- Verify and update relevant member information.
- Complete assessment of lifestyle, wellness, and SDoH related topics.
Documentation
- Documents Coaching outcomes including Goal attainment, and barriers to change.
- Appropriately documents outreach attempts, assessments, follow-up notes, planning and member interactions in CCA system.
Other
- Build strong relationships and infrastructures that designate Mass General Brigham Health Plan as a people-first organization.

Qualifications
- Experience working with patients/health plan members with SDoH needs required.
- RN or RD/N or LDN preferred.
- Master's in a health-related field preferred.
- Bilingual (English/Spanish) preferred.
- Wellcoaches School of Coaching certification preferred.
- Association of Diabetes Care and Education Specialists Lifestyle Coach Certificate required or earn a certificate within first 12 months of employment. Candidates may also submit certification from another organization with a signed Memorandum of Understanding (MOU) with the CDC to provide training.
- Bachelor’s degree in a health-related field required or the equivalent combination of training and experience, plus 3 years of related experience in behavior change counseling, health coaching, or care management.
