48 hours every two weeks
Under the supervision of a Home Health Services RN, share a patient caseload to achieve desired patient outcome. Including coordinating patient care and contacting physicians as appropriate, providing patient education, screening daily patient transmissions. This position must be able to demonstrate the knowledge and skills necessary to provide service appropriate to the age of the patients served in the home health environment.
They are an integral part of preventing hospitalizations and Emergency Room visits for patients, facilitating care in the community through communication with physician offices, community resources, patients and families.

Promotes patient safety by maintaining the policies and procedures of the hospital and nursing department, including maintaining the Patient's Bill of Rights. Understand own work area professional concepts/standards, regulations, strategies and operating standards. Quality of Practice
Systematically enhances the quality and effectiveness of nursing practice. Makes decisions regarding own work approach/priorities and follows direction.
Prepare, submit and maintain documentation as required by the organization.
Visit/shift notes documented on day services are rendered
Education
Attains knowledge and competency that reflects current evidence-based nursing practice.
Professional Practice Evaluation
Evaluates one's own nursing practice in relation to professional practice standards and guidelines, relevant statutes, rules and regulations.
Collaboration
Collaborates with patient, family and other members of the healthcare team in the conduct of nursing practice. Confer with the RN Supervisor regarding needed changes in the Plan of Treatment
Ethics
Integrates ethical provisions in all areas of practice.
Environmental Health
Practices in an environmentally safe and healthy manner.
