Job Description
Trinity Integrated Care is seeking a compassionate and experienced Group Facilitator for our
Behavioral Health facility. The Group Facilitator will be responsible for running psychoeducation
groups, substance groups, and coordinating care with members' clinical teams. The ideal
candidate must be comfortable taking copious notes and understanding treatment plans. They
must also be comfortable using an electronic medical record (EMR) system with ease.
Responsibilities:
  • Run psychoeducation groups and substance groups in accordance with best practices
and evidence-based treatment modalities
  • Collaborate with members' clinical teams to ensure coordinated and comprehensive
care
  • Facilitate group discussions and activities that promote member engagement and
progress towards treatment goals
  • Develop and maintain a therapeutic environment that is safe, supportive, and conducive
to healing
  • Take copious notes on member progress and maintain accurate and up-to-date records
in the EMR system
  • Participate in treatment planning meetings and provide input on member goals and
interventions
  • Provide feedback to members on their progress towards treatment goals and offer
support and encouragement
Qualifications:
  • At least 2-3 years of experience running psychoeducation groups and substance groups
in a behavioral health setting
  • Experience working collaboratively with clinical teams and coordinating care
  • Ability to take copious notes and maintain accurate and up-to-date records in an EMR
system
  • Strong organizational and analytical skills
  • Knowledge of evidence-based treatment modalities and group facilitation techniques
