OPEN POSITION: - Nurse Practitioner, Geriatric Primary Care - Skilled Nursing Facilities (SNFs) SCHEDULE: - Full-Time - Monday through Friday - No Nights or Weekends - Some Flexibility Offered COMPENSATION: - $100,000 to $120,000 Base Salary, dependent upon experience - Regular Bonuses - Malpractice Insurance - Health / Dental / Vision Insurance - Life / AD&D / STD / LTD Insurance - FSA and HSA Plans - Paid Time Off Package - CME / Licensing Allowances - 401k w/ Employer Match - Many Other Perks and Benefits LOCATION: Terre Haute, Indiana COMPANY PROFILE: This organization is a world-class provider of geriatric medical services, delivering personalized care to thousands of skilled nursing facility (SNF) residents every day across multiple regions. They have a proactive, preventative approach to healthcare that uses evidence-based research to improve the lives of patients and effectively prevent hospital admissions. They are currently hiring due to growth in their Terre Haute market. POSITION DESCRIPTION: The nurse practitioner is the on-site provider in the skilled nursing facility, regularly rounding on patients and responding to urgent needs. Providers are typically assigned to 1 or 2 facilities and only visit one (1) location per day. The organization believes strongly in a proactive approach to medicine, with a clinical focus on: - preventative care - chronic disease management - medication management - routine geriatric primary care - expedient response to urgent needs and changes in condition The practice uses the GEHRIMED electronic health record (a mobile-based platform specialized to this type of setting). On-call duties are minimal and shared evenly among the providers; the rotation is typically every 4-5 months and call volume is light.
