Highlights: Position Title: File Clerks Location: 220 Richmond Ave, Batavia, NY 14020 Duration: 2 months Work Schedule: Day Shift Responsibilities for this position include, but are not limited to:
- Tracks, investigates, and reports privacy issues and breaches of Protected Health Information.
- Participates in data collection and presentation for clinical and administrative review.
- Provides assistance to clinical staff on health records policies and procedures and entering proper data into health records.
- Corresponds with residents and resident families, and other entities regarding health record information in a confidential manner following all HIPAA policies and procedures.
- Organizes, manages, and analyzes medical records to ensure quality, security, accessibility, and compliance with accreditation and legal requirements.
- Implements legal and regulatory policies, procedures, and requirements for access and disclosure of Protected Health Information.
- Assists facility staff with resolving problems of medical records maintenance and establishing acceptable terminology in all related documentation.
- Monitors facility compliance with respect to the confidentiality of resident information and complies with HIPAA policies and procedures.
- Reviews resident medical records to ensure required components are in place such as clinical progress notes, laboratory reports, and treatment records.
- Maintains databases and tracks all resident health information disclosures and ensures timely completion of requests.
- Ensures corrective action of internal and external audit findings is completed.
- Identifies missing information and informs the appropriate discipline responsible for the documentation of discrepancies.
- Reviews diagnostic and therapeutic medical records for completeness and accuracy.
- Coordinates processes required for Medicaid funding and ensures proper clinical and medical documentation is complete and accurate.
- Provides technical assistance, training, and support to facility and clinical staff on health information management requirements, including diagnostic and procedural coding, to meet local, State, and federal guidelines.
