About the Position:
This position is located in the Department’s Services to End and Prevent Homelessness division. The vision of Services to End and Prevent Homelessness (SEPH) is a home for everyone in Montgomery County, where the experience of homelessness is rare, brief, and one-time only. The mission is to strive to be a leader in the work to end homelessness across our nation. Through commitment and collaboration, SEPH seeks to create effective systems to achieve the vision. The housing and homeless continuum embraces a Housing First philosophy. Housing First recognizes that people are most successful when they have choice in housing and seeks to eliminate barriers such as sobriety requirements or treatment compliance. SEPH provides a full continuum of services including housing stabilization, homeless diversion, and permanent housing; and employs evidence-based and promising practices. The mission cannot be achieved without collaborating with public and private partners through the Interagency Commission on Homelessness. Special needs populations include veterans, both individuals and families, persons with significant medical and behavioral health challenges, individuals with developmental disabilities, and transitioning youth and seniors with disabilities experiencing or at risk of homelessness.
The Homeless Health Community Health Nurse II (CHN II) will:
- Complete initial assessments and triaging of homeless clients (unsheltered) and previously homeless (permanent supportive housing), provide psychiatric support and assessment as it relates to medication compliance and needed follow-up, participate in hospital discharge planning for homeless and previously homeless clients transitioning from the hospital back to the community while ensuring an appropriate safe discharge with after care plan
- Provide health screening for various medical conditions health education, medication management and monitoring
- Participate as an active team member of an outreach team to medically and psychiatrically vulnerable clients throughout Montgomery County
- Administer medications and injections-as prescribed by the prescribing physician
- Implement relapse prevention strategies for mental and physical health and other service interventions to ensure client's stability and be available as a supportive medical resource to the clients and other members of the treatment team.
- Be a part of a multi-disciplinary clinical team that will provide recovery through community treatment and habilitation to clients who need ongoing supportive contact to maintain prior to and after being housed. Individuals/families may be in emergency or transitional shelters, hospitals, rehab centers, correctional facilities, permanent supportive housing or street homeless (unsheltered)
- Provide time limited, medically focused nursing case management services to homeless individuals/families in the community and previously homeless clients in the home.
Bilingual applicants in the English/Spanish languages are encouraged to apply. If the candidate selected for this position possesses multilingual skills, he/she will be required to successfully pass an examination assessing oral communication, reading comprehension, and interpreting skills for Basic proficiency. For Advanced proficiency, the candidate must also pass an examination requiring translating text from the selected language to English and vice versa.
The selected candidate will be required to successfully pass a pre-employment physical exam, drug and alcohol testing.
