Posting Summary

The “HealthyWe: Keeping a Healthy Weight” program is a grant funded initiative of the Rutgers School of Nursing Camden funded by the New Jersey Department of Health Division of Community Health Services. HealthyWe is an essential component of The-Optimal-Lymph-Flow ( TOLF ) program that leads to improvement in weight, pain, activities of daily living, and mental health (anxiety and depression) among breast cancer patients and heart failure patients who also face similar personal barriers as older adult residents in Camden.

To build community capacity for improving weight and reducing obesity, we will engage three (3) “Champion Residents” from the communities to be trained as Community Coaches for implementing the HealthyWE program; a strategy to sustain the TOLF program in the communities.

Community coaches will facilitate obesity reduction activities facilitated at community sites in Camden City, which include;

  • Engagement with 15-20 older adults (age 60 and older) that are recruited to participate in the HealthyWe program.
  • Participation in weekly check-ins with the project director about challenges and successes related to the project implementation.
  • Regular check-ins with the participants in the program over the phone or in person.
