Description:


POSITION PURPOSE


The certified nurse assistant is responsible for providing various care services necessary for attending to the personalneeds, support, and health status of participants.

Essential Functions


  • Execute assignments in a culture that is shared and collaborative across all divisions within TH PACE.
  • Consistently demonstrate (leads by example) TH PACE Values to all internal and external customers(Participants, visitors, volunteers, and colleagues). Create and maintain an atmosphere of warmth, propagating acalm environment throughout the office.
  • Assure compliance with infection control, personal protection equipment, blood-borne pathogens, hazardousmaterial handling, and fire safety standards.
  • Maintains a working knowledge of applicable Federal, State, and local laws and regulations, TH PACE CorporateIntegrity Program, Code of Ethics, as well as other policies and procedures in order to ensure adherence in amanner that reflects honest, ethical and professional behavior.
  • Responsible for participant safety, identify safety hazards and emergency situations, and initiate corrective actionas able.
  • Escort participants to medical, dental, optometry, rehabilitation, and other appointments as necessary.
  • Attend and participate in scheduled training, in-service training, mandatory annual in-service training andeducational classes as required/needed.
  • Assist transportation staff with loading and unloading of participants from vehicles. Assist participants being

  • As part of the health care team, provide personal care assistance that will maintain the independence of theparticipant to conduct activities of daily living, including toileting, showers, eating, and grooming. Assist in themaintenance of physical activity, exercise, and walking.
  • Participate as a member of the interdisciplinary team in ongoing participant assessment and monitoring. Reporton personal care services provided and the self-care abilities of participant, including changes noted inparticipant’s condition.
  • Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow throughwith nursing and medication instruction, diet restrictions, and emotional reassurance.
  • Know, understand, incorporate, and demonstrate the Mission, Vision, Values and Guiding Behaviors of TrinityHealth and Trinity Health PACE (TH PACE) in behaviors, practices, and decisions.
  • Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by yoursupervisors. Follow work assignments, and/or work schedules in completing and performing your assigned tasks.
  • Promote and maintain collaborative relationships with managers, peers, and customers by effectively fostering ateam environment, building consensus and resolving conflicts.
  • Assist in identifying signs and symptoms indicating physical, emotional, or behavioral change in participants andreporting these signs to the center staff and/or supervisor, including but not limited to, any changes in skin integrity, bowel and bladder status, food/fluid intake, or changes in ambulation.
  • Provide nursing care as assigned in a safe manner, consistent with the policies and procedures of the PACEOrganization.

Minimum Qualifications


  • Comprehensive to expert proficiency with Microsoft product suite (MS Word, Excel, Power Point, etc.); basicknowledge and experience with electronic mail and calendaring system. Knowledge of systems necessary,experience preferred. Ability to type with speed and accuracy. Ability to use other software as required to performthe essential functions of the job.
  • Must be action-oriented, have business acumen, manage conflict well, be customer focused, have high decisionquality, flexibility to adapt to ongoing change and have organizational agility. Ability to work with minimalsupervision and exercise independent judgment.
  • Possesses a high degree of personal accountability, responsibility and independent decision-making abilities withthe skills to interpret programs, goals, objectives, policies and procedures of the organization in line with mission,vision, and philosophy of TH PACE.
  • Position may require 10% travel within the Centers geographic region and use of personal vehicle.
  • Demonstrates superior written and verbal communication and presentation skills appropriate for audiencecomprehension. Well-developed communication skills, both written and oral, that may be used either in an on-siteor virtual environment is required. Able to communicate effectively with individuals and groups representingdiverse perspectives.
  • Experience in a long-term care facility or with a community-based geriatric program preferred.
  • High School Diploma or equivalent required. Must be an active certified nurse assistant in the state where PACEOrganization is located.
  • State of North Carolina - Must have Employee Medical Statement that meets the requirements of the Standardscompleted prior to beginning employment, signed within previous 12 months by an MD, NP or PA, indicatingthat the employee has no illness or health condition that would pose a risk to others and can perform the dutiesassigned in the job.
  • Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one(1) year of experience, must receive appropriate training from the PACE organization on working with a frail orelderly population upon hiring.
  • Ability to research, analyze and assimilate information from various on-site or virtual sources based on technicaland experience-based knowledge. Must exhibit critical thinking skills and possess the ability to prioritize.
  • Excellent organizational skills. Ability to perform multiple duties and functions related to daily operations andmaintain excellent customer service skills. Ability to perform frequent detailed tasks and provide immediateservice with frequent interruptions. Ability to change and be flexible with work priorities. Strong problem-solvingskills.


PHYSICAL AND MENTAL REQUIRMENTS AND WORKING CONDITIONS


  • Must be able to adapt to frequently changing work parameters and adapt to work priorities that frequently change.


Must be able to work on a variety of tasks/projects in physical or virtual environments that may be stressful withindividuals having diverse personalities and work styles.


  • Must be able to see and hear or use prosthetics that will enable these senses to function adequately so that therequirements of this position can be fully met.
  • Primarily works inside with frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting.


Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.

  • May be required to work beyond normal working hours, on weekends and other positions temporarily whennecessary and may be subject to call back during emergency conditions. May be required to work on shifts otherthen those which originally hired.
