Job Description

In this position the candidate carries out all assigned para-professional nursing care and treatment services in prescribed and efficient manner. Provides a safe, therapeutic and hygienic environment. Learns, implements and monitors Arizona Management System (AMS) methods. Incorporates a cultural focus on Veteran/Member-centered care in the performance of job duties.

Job Duties:
  • Informs dining staff about Veterans'/Members' choices and picks up food from dining staff.
  • Turns and repositions Veterans/Members as directed.
  • Responds appropriately to licensed nurses and assists with assaultive or combative residents following established policies and procedures.
  • May use a variety of medical equipment (e.g., lifts, oxygen, geri-chairs, braces, prosthetic devices, catheter care, etc.) in accordance with physician orders, licensed nurse direction and/or care plan.
  • Listens to and communicates resident requests/concerns as appropriate.
  • Participates fully in the QAPI process, our Quality Assurance and Performance Improvement program mandate by CMS which incorporates AMS.
  • Completes special projects as assigned.
  • Actively involved in AMS processes such Huddle Boards, Problem Solving, Job Breakdown Sheets, Success Tracking, and Standardized Work.
  • Follows instructions to use simple procedures and devices to strengthen Veterans'/Members' limbs and maintain flexibility.
  • Maintains record of Veteran/Member food consumption.
  • Always utilizes universal precautions.
  • Is responsible for medical record and worksheet documentation on all assigned Veterans/Members.
  • Assists the licensed nursing staff and Veteran/Member by performing a variety of care tasks.
  • Assists nursing staff with simple medical procedures such as suppositories, enemas, treatments, post surgical care and post-mortem care.
  • Reports to and assists licensed nurses in emergency situations rapidly and appropriately following established policies and procedures.
  • Informs them about menu choices available for the day.
  • Helps Veterans/Members with transfers, ambulation, eating, bathing, dressing, toileting, personal hygiene and other ADLs.
  • Transports Veterans/Members to the dining room.
  • Keeps records and prepares simple reports.
  • Communicates and documents all information as appropriate.
  • Clears soiled items from table after each course and places them in assigned location.
  • Serves Veterans/Members their meals in courses; assists them if necessary.

Knowledge of:

  • Practices and techniques applied in maintaining a safe, orderly, therapeutic and hygienic environment
  • State and agency laws and regulations governing para-professional nursing practices
  • Practices and procedures of using medical equipment in assigned duties
  • Practices and procedures established for the work environment
  • Principles, practices and techniques of para-professional nursing care, treatment and services appropriate to the work setting

Selective Preference(s):
  • 1+ years in a Skilled Nursing / Long-term Care facility
Pre-Employment Requirements:
  • Active certification to practice as a Certified Nursing Assistant in the State of Arizona
  • Current Negative TB skin test (Mantoux skin test) with in the last 6 months or a written statement from a physician, physician’s assistant or a registered nurse practitioner indicating freedom from tuberculosis, if in the past has had a positive skin test or tuberculosis
  • This position is subject to the vaccination requirements outlined in CMS Interim Final Rule - Medicare and Medicaid Programs; Omnibus COVID-19 Health Care Staff Vaccination (published November 5, 2021) (86 FR 61555)
  • Fingerprint clearance card issued by the Arizona Department of Public Safety
