POSITION SUMMARY
The team requires a highly motivated, energetic, and dynamic individual that will be part the clinical services (Refill Protocols and Telehealth) within UH Meds. The Clinical Pharmacist is responsible for the safe, effective, and fiscally responsible use of medications through the application of clinical knowledge, skills, and professional judgment. The Clinical Pharmacist serves as a patient advocate when planning and implementing pharmacy care by considering each patient’s disease states and background. These activities occur in all areas of pharmacy practice throughout the enterprise. The Clinical Pharmacist is legally responsible for the review and approval of all activities related to the dispensing of patient specific medications pursuant to a valid prescription or medication order from a practitioner as defined in the policy.
REPORTING RELATIONSHIP
This position will report directly to the UH Meds Pharmacy Services, Clinical Operations Coordinator.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The responsibilities of the Clinical Pharmacist include but is not limited to the following:
- Collaborate with pharmacy resident rotation
- Guides the orientation and training of new and existing professional and technical staff for their specific job descriptions
- Develops core competencies and appropriate documentation
- Compile meaningful productivity metrics
- Monitors compliance with policies and continuous quality improvement of pharmacy services
- Participate in the educational activities for medical, nursing and pharmacy staffs
- Participates in development of new pharmacy services
- Collaborate with providers on prescription protocols
- Collaborate with the providers to identify gaps in care
- Responsible for coordination of the activities of professional & technical employees engaged in ensuring safe, efficacious, and cost-effective preparation and/or use of medications
- Support and implement programs developed by MS&T, SMS&T and other relevant committees
- Assures adequate and proper staffing and scheduling
- Assists in regulatory compliance by developing policies for operations in accordance with laws and regulations
- BCPS Certification Preferred
- BS of Pharmacy or Pharm D degree
- Licensed pharmacist in the State of Ohio (Registered Pharmacist)
- Completion of Accredited Residency Preferred
