This is an exciting opportunity to shape the continued development of our care model and drive the expansion and build-out of our Primary Care Physician Clinic platform. We are looking for a dynamic, experienced health care delivery leader who loves motivating others and thrives on fast-paced change. The Regional Practice Manager will lead our current primary care physician clinics within Southwest Florida and be the primary driver of continued expansion. This position oversees clinics in the Southwest Florida area and involves extensive local travel.


In partnership with the Regional Managers the Clinical Navigator


  • Provides oversight and support to providers and clinical teams in order to improve patient and clinical outcome goals while following established clinical standards and practices.
  • Proactively identifies opportunities for improvement in patient care outcomes and clinical workflows
  • Collaborates to support appropriate individualized patient care.
  • Ensures the quality of clinical services provided meets organizational care standards.
  • Encourages and oversights a high level of provider and clinic team engagement
  • Ensures all assigned providers and clinic teams are up to date with individual patient case information


Key Responsibilities


  • Supports the MA IV certification, phlebotomy training initiative workflow as needed
  • Partners with PhyPartners Health Services support initiatives as appropriate to improve VIP outcomes
  • Oversights the ongoing identification, care, and documentation for all transplant, lifevest, dialysis, worry, and opportunity patients
  • Provides clinical setup for new clinics, clinic acquisitions, and clinic relocations as necessary
  • Identifies resources for patient services that do not meet medical necessity criteria
  • Works with assigned providers and clinic teams to support biomedical equipment requests, repairs, and training
  • Monitors clinical grievance/complaint process to ensure informed determinations are made and timely responses are provided
  • Partners with Regional counterparts to conduct regularly scheduled (weekly, monthly, etc.) provider and clinic team follow-ups, onsite visits, and meetings with a focus on evaluating for progress/regress, promoting continuity of care, and ensuring improved health outcomes via established Care Navigation and clinical ops workflows and programs
  • Connects assigned providers and clinic teams with health plans and/or community resources for patient support
  • Facilitates standardized clinical compliance/quality processes, training and accountability tracking including but not limited to the completion of all safety logs, clinical logs, and clinical workflows
  • Provides Care Navigation outreach program training in order to develop clinic workflows around care coordination
  • Oversights assigned clinic teams admission management and tracking (TCM/CCM) processes
  • Provides Care Navigation and care coordination onboarding introductions for new team members
  • Oversights appropriate clinic team documentation of care management and clinical activities
  • Supports appropriate procurement/maintenance of clinical supply inventory


Qualifications Required


  • Bilingual in Spanish is a plus but not required
  • Clinically sound - outpatient clinic-based experience preferred
  • Ability to lead and influence positive change
  • Proven efficiency with clinical standards and oversight
  • Possess and maintain a current, valid driver’s license
  • Proactive approach and dedication to standards, integrity, and ethical practices
  • Proficient in generating reporting and record-keeping procedures (Proficiency in Google Suite)
  • MUST reside in the state of FL
  • An ability to maintain composure in demanding or stressful situations
  • Methodical and diligent with outstanding planning abilities
  • 3+ Years of experience working as a Medical Assistant (or equivalent) in an outpatient setting - primary care preferred
  • Comfortable teaching and educating teams and individuals on clinical topics
  • Excellent organizational skills and detail-oriented
  • An analytical mind able to “see” the complexities of procedures
  • Ability to communicate effectively with providers and clinic teams
  • Ability to coach and train others with a support minded approach
  • Experienced in health administration, compliance, and risk management a plus
  • A clinical license or certification is a plus
  • Resourceful in solving complex patient care opportunities
  • In-depth knowledge of current industry standards including OSHA, Biohazard, CLIA, etc.
  • Proficient with Healthcare Management Systems and Microsoft Office Word and Excel
  • Excellent communication skills, both written and verbal
  • Ability to travel to assigned locations as needed
  • Ability to operate in a fast-paced and engaging environment.
