Overview

Teen Health Connection is looking for a Patient Nurse Navigator to join our team full-time to work primarily with the Center for Gender Health.

The ideal candidate is an experienced RN, who is passionate about providing care to our patients. This position combines hands-on patient care with administrative components. Experience working with prior authorizations and working knowledge of specialty services or pediatrics is preferred. This position will also be cross trained to work in clinic for our other specialty programs: Center for Disordered Eating and Teen & Tots program.

Levine Children's Center for Gender Health is the first gender health clinic of its kind in Charlotte and one of only a handful in the region. Our care supports every aspect of our patients' gender identity - in a setting that helps them feel welcome and accepted for who they are.

Established in 1992, Teen Health Connection is a nonprofit healthcare practice that provides comprehensive adolescent medicine and behavioral healthcare and health education services for adolescents ages 11 to 25. In addition to providing comprehensive medical care, Teen Health Connection works to promote healthy lifestyle and behavioral choices through community-based prevention and health education programs. Program initiatives include: pregnancy prevention; substance abuse prevention (Teen Advisory Board/Youth Drug Free Coalition); and the Empower: Teen Leadership Summit. A variety of parenting classes are offered free of charge to help parents raise responsible teens.

Mission: To improve the health of adolescents by providing adolescent medicine and behavioral health services, education, advocacy, leadership development, and research through connections among adolescents, parents, and the community.

Shift Details: Monday - Friday 8am - 5pm


Job Summary

Coordinate the patient's care to increase the efficiency and quality of care that is delivered to the patient. This role will start with the time of the patient's diagnosis and will continue through the patient's journey throughout their care. Will be the patient advocate and care for them in a highly supportive and personal manner and also will serve as a liaison to ensure that patients and families receive resources and services as needed. The Patient Navigator provides professional nursing care through skillful assessment, diagnosis, outcome identification, planning, implementation, and evaluation in the ambulatory, outpatient, and Medical Group practice settings in accordance with policies and procedures under the supervision of the Clinical Supervisor and Manager. The Patient Nurse Navigator will assist in maintaining a safe work environment and perform all related job responsibilities in a safe manner. Maintains clinical and professional competency as appropriate to the population of patients served.


Essential Functions

  • Practices in a manner that is congruent with cultural diversity and inclusion principles.
  • Assesses every new patient for emotional and social needs as well as barriers to care.
  • Provides language specific education, psychosocial support, and necessary resources for new and existing patients, and their families, and coordinates appropriate referrals for those needed services.
  • Communicates effectively in all areas of practice.
  • Educates and provides information to new and existing patients and families regarding diagnosis, work-up, treatments to include injectable and/or oral medications, therapy, support services, financial issues, community resources, and follow-up.
  • Serves as liaison for patients and families.
  • Utilizes appropriate resources to plan, provide, and sustain evidence-based nursing services that are safe, effective, and fiscally responsible.
  • Contributes to quality nursing practice and collaborates with other key stakeholders.
  • Ensures and coordinates (where appropriate) new patient referrals, scheduling of consultations, procedures and appointments, identifies potential gaps as well as the coordination of care for patients returning to their communities for treatment and follow-up.
  • Identifies trend barriers to patient expedient care and will conduct service recovery as needed. Responsible for patient satisfaction.

Physical Requirements
Ability to lift objects weighing up to 25 pounds. Occasional stooping, kneeling, crouching. May require sitting/standing for long periods of time.


Education, Experience and Certifications

BSN Degree Required/MSN Preferred. Current licensure to practice as a Registered Nurse in North Carolina required. Certification in the appropriate specialized field within 2 years of being in the position required. Excellent communication skills required.
