Job Description
We are looking for a dedicated Childrens Care Coordinator to assist us with patients at our Seaford, Delaware Woman's Residential facility. A Childrens Care Coordinator’s responsibilities include consulting with patients and determining their needs, developing an individualized plan of action that includes coordination from all parties involved in order to provide the best possible patient care. Ultimately, you will have a compassionate nature, be knowledgeable about health care practices and provide exceptional customer service to assist patients in handling their illness. The Childrens Care Coordinator has primary responsibility for providing quality, sufficient and appropriate care management and advocacy services to the clients served, with a specialized focus on children’s services. A Children's Care Coordinator does this by increasing access to care through outreach and enrollment, improving health through education and support, and appropriate use of health care and behavioral health services. All services are performed on-site and in the community. Requires local travel within the community.


Please note this is a full-time Monday-Friday day position with occasional evenings and weekends on-call.


  • $1,500 sign-on bonus ***


Responsibilities


  • Conducts individualized care management needs assessment, in partnership with the care team assigned to the parents, prioritizing the data collection based on the client’s immediate condition or needs within 72 hours.
  • Respond to calls from parents, caregivers, providers and professionals pertaining to development, behavior or learning for children
  • Identify services needed for the child(ren) and assist parents in connecting with services. This includes all aspects of coordinating care (transportation, scheduling etc)
  • Proving cultural mediation between the parents and health and human services systems.
  • Conduct periodic updates on health status of children according to program guidelines.
  • Educate and counsel parents about plans of care, health promotion and disease prevention.
  • Attend all assessments and services relevant to the child(ren).
  • Support in the processing of completed developmental screenings
  • Assists with maintaining developmental services database
  • Educate client and children in an age-appropriate manner.
  • Coordinates and consults with Child Development Community Liaisons to identify appropriate referrals and services
  • Participate in weekly case conference and monthly team meetings to ensure care coordination.
  • Performs any other duties assigned by leadership team.


Requirements


Education and Certification:


  • CADC preferred but not required
  • Bachelor’s Degree in Child Development, Early Childhood Education, Human Services, Social Work or related field


Experience:


  • Proven work experience as a Care Coordinator or similar role
  • 2 years experience working with children in a behavioral health setting
  • Specialized training or experience in women’s studies/services child development and trauma, public health practices, disease prevention, and healthcare systems


Skills:


  • Strong customer service skills
  • Highly organized with a strong ability to manage different priorities
  • Apply good judgment to fast-changing situations
  • Ability to work with others and use resources to problem solve
  • Effective verbal and written communication skills
