Job Description
We are looking for a dedicated Case Manager to assist us with Children and Family care coordination at our Seaford, Delaware Woman's Residential facility. The Case Manager - Children & Family Services will be responsible for consulting with patients and determining their needs, developing an individualized plan of action that includes coordination from all parties involved in order to provide the best possible patient care. Ultimately, you will have a compassionate nature, be knowledgeable about health care practices and provide exceptional customer service to assist patients in handling their illness. The Case Manager - Children & Family Services has primary responsibility for providing quality, sufficient and appropriate care management and advocacy services to the clients served, with a specialized focus on children’s services. A Case Manager - Children & Family Services does this by increasing access to care through outreach and enrollment, improving health through education and support, and appropriate use of health care and behavioral health services. All services are performed on-site and in the community. Requires local travel within the community.


Please note this is a full-time Monday-Friday day position with occasional evenings and weekends on-call.

Responsibilities


  • Performs any other duties assigned by leadership team.
  • Conducts individualized care management needs assessment, in partnership with the care team assigned to the parents, prioritizing the data collection based on the client’s immediate condition or needs within 72 hours.
  • Attend all assessments and services relevant to the child(ren).
  • Educate and counsel parents about plans of care, health promotion and disease prevention.
  • Coordinates and consults with Child Development Community Liaisons to identify appropriate referrals and services
  • Assists with maintaining developmental services database
  • Conduct periodic updates on health status of children according to program guidelines.
  • Proving cultural mediation between the parents and health and human services systems.
  • Respond to calls from parents, caregivers, providers and professionals pertaining to development, behavior or learning for children
  • Participate in weekly case conference and monthly team meetings to ensure care coordination.
  • Support in the processing of completed developmental screenings
  • Identify services needed for the child(ren) and assist parents in connecting with services. This includes all aspects of coordinating care (transportation, scheduling etc)
  • Educate client and children in an age-appropriate manner.


Requirements


Education and Certification:


  • Bachelor’s Degree in Child Development, Early Childhood Education, Human Services, Social Work or related field
  • CADC preferred but not required


Experience:


  • 2 years experience working with children in a behavioral health setting
  • Proven work experience as a Care Coordinator or similar role
  • Specialized training or experience in women’s studies/services child development and trauma, public health practices, disease prevention, and healthcare systems


Skills:


  • Effective verbal and written communication skills
  • Apply good judgment to fast-changing situations
  • Highly organized with a strong ability to manage different priorities
  • Ability to work with others and use resources to problem solve
