River Edge is currently seeking a Full-Time Case Manager at the Bibb County Sheriff's Office for the Judiciary In-reach Program.


Location/ Schedule: 668 Oglethorpe St, Macon, GA 31201


Role Summary


Under the supervision of the Program Manager, the Case Manager will provide various service needs (mental health, housing, employment, etc.) that require assistance from multiple providers. Case managers also provide counseling and coordinate services during incarceration and refer detainees from the Bibb County Sheriff's Office (BCSO) to community- based providers for services after release.


Duties And Responsibilities


  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and detainee centered.
  • Other assigned duties
  • Evaluating the goals and needs of detainees to develop targeted treatment plans for those undergoing issues mental illness.
  • Help detainees make informed decisions by acting as their advocate regarding their clinical status and treatment options.
  • Planning for crises and helping detainees develop coping mechanisms.
  • Building after-care treatment plans and communicating them to detainees for use once they're released from treatments.
  • Prepares and submits reports to Supervisor or Team Lead as required.
  • Maintain complete and organized files.
  • Assists detainees in locating and utilizing community resources including legal, medical, financial assistance.


Qualifications


  • All applicants must pass a satisfactory background clearance and pre-employment drug test.
  • Must have excellent communications skills, both verbal and written.
  • Provide 2 Professional references (Former employers only, no family members)
  • Bachelor's degree in social service field (Social Work, Sociology, Psychology)
  • Must have ability to multi-task and remain organized.
  • Must have knowledge and experience with the chronically mentally ill population.
