Case Manager
EAC Network, a not-for-profit social service agency that empowers, assists, and cares for over 65,000 people in need through 100 programs across Long Island and NYC, seeks a Case Manager to work part time for its Services for Minor Victims program.
The Case Manager position is a non-exempt position paying $25 per hour. The position is part time, up to 28 hours per week. The office is located in Central Islip, NY and the role reports to the Program Supervisor.

Primary Purpose of Job:
The Safe Harbour Case Manager is responsible to provide short and long term, trauma informed, case management and direct services for minor victims of human trafficking and youth at-risk of human trafficking that are referred to Safe Harbour. The Safe Harbour Case Manager is a service provider under the Services for Minor Victim of Sex Trafficking grant with probation and will coordinate with additional human services agencies as needed.

Principal Duties & Responsibilities:
  • Complete risk-assessments for minor victims of human trafficking and youth at-risk of human trafficking that are referred to Safe Harbour.
  • Provide trauma informed, victim centered case management services and outreach to minor victims of human trafficking and youth at-risk of human trafficking to youth in Suffolk County, NY.
  • Conduct assessments for client needs and develop an individualized service plan to support clients with safety planning, goal setting and psychoeducation.
  • Document in the CAC NYSCA database the services provided to the client to ensure accurate case records are maintained.
  • Collaborate with community partner agencies to deliver staff training regarding the identification of unaccompanied minors at risk of trafficking and connect then to services.
  • Transport and escort clients to court proceeding, medical appointments and other appointments as needed.
  • Assist with applying for public benefits, employment, medical care, return of personal property and short- and long-term coordination of such services.
  • Make appropriate referrals for medical exams, mental health services, residential assistance.
  • Maintain accurate database of professional referral resources that assist stated population.
  • Prepare monthly and quarterly statistical and annual reports as required by funding source.
  • Attend workshops and trainings as need for project implementation.
  • Assist in preparing reports for the grant as needed.
  • Participate in team meetings, case conferences, agency presentations, trainings and related events.
Knowledge, Skills, & Abilities Required:
  • Bachelor’s Degree is required, a degree in Mental Health Counseling, Social Work, Forensic Psychology or Behavioral Health field preferred.
  • Minimum of two years of relevant experience.
  • Must be proficient in computer applications or other automated systems such as Google Drive, spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.
  • Excellent communication, organizational and interpersonal skills.
  • Must have access to automobile and possess valid NYS driver’s license.
