At Mesquite Rehabilitation Institute, we are passionate about patient care! Nationally Recognized Our hospital has been ranked in the Top 10% nationally for rehabilitative care for 11 years and has earned The Joint Commission’s Advanced Stroke Certification and Hi Fracture Certification. At our 30-bed hospital, we provide specialized inpatient and outpatient rehabilitative services to our patients, serving Mesquite and the surrounding communities. We see patients who often are recovering from disabilities caused by injuries, illnesses, or chronic medical conditions.

We are seeking a Case Manager to join our team!

Works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. Ensures patient is progressing towards desired outcomes by monitoring care through assessments and/or patient records. Identifies and resolves barriers that hinder effective patient care. Actively involved in discharge planning process. Integrates the hospital’s mission and “Guiding Principles” into daily practice.

Required Skills

• One year recent relevant experience required. Acute care setting preferred.
• Current clinical licensure strongly preferred.

• Ability to maintain quality, safety, and/or infection control standards.
