Job Description
CARE of Southeastern MI is currently hiring a full-time Case Manager - Family Based.


Duties And Responsibilities


  • Maintain a case load and deliver services.
  • Implements and monitors treatment plans that are developed with a Person-centered/Family-centered philosophy.
  • Provide in-home parenting support and facilitate parenting lessons.
  • Responsible for the identification of treatment referrals/resources within the community appropriate to meeting the needs of the individuals served.
  • Conducts screenings and comprehensive assessments for early childhood issues and co-occurring behavioral health issues according to best practices and agency policy and procedures.


Education


  • A Bachelor's degree in social work, sociology, psychology, family ecology, consumer/community services, family studies, family and/or child development, guidance/school counseling, counseling psychology, education, public health, human services, or criminal justice.


Skills And Experience


  • Must possess knowledge and understanding of childhood development milestones and trauma informed services.
  • Lived experience as a parent or caregiver is preferred.
  • Experience working with early childhood issues or related field.
  • Ability to work independently as well as in a team environment.
