Job Description

In our Youth and Family Services, we help parents and young people who are striving to become healthy and restore their relationships. We work in partnership with human services, child welfare systems, juvenile courts, and community-based service providers so that together we can help individuals and families obtain the services and skills they need to live in positive, healthy ways.


Summary: This position will assist with the coordination of services for client care and maintaining required documentation. position is responsible for bringing services, agencies, resources and other people together to assist clients in achieving established goals. This position may also work with high risk caseloads, co-occurring clients, families and children.


Essential Duties And Responsibilities


  • Provide referrals for identified needs of the client and crisis intervention when needed.
  • Review and maintenance of medical records as needed.
  • Maintain effective working relationship with other area service providers.
  • Facilitate individual or group orientation or educational sessions (as required).
  • Complete and maintain all required documentation pertaining to clients (Cornerstone).

  • Provide case management to coordinate other services and resources.
  • Provide specialized case management services to clients and their families with needs.
  • Screen and assess client for their appropriateness for agency services.


Qualifications/Requirements

  • Knowledge of the Health Insurance Portability and Accountability act(HIPPA)
  • Knowledge of computers, including proficiency in Microsoft Office applications and the internet
  • Ability to work with a diverse population
  • Possess clinical skills to effectively manage clients
  • Ability to work flexible hours and travel as needed (evening/weekend hours)
  • Specific knowledge necessary for working with special population
  • A valid RN license through the state of Illinois Department of Financial and Professional Regulations.
  • Bachelor’s degree in nursing and/or valid RN license; one year experience in community health or maternal and child nursing related activities; or Masters in social work with equivalent combination of training and experience that provides the following knowledge abilities and skill.
  • Must be available to attend meetings and meet the needs of the program and/or clients throughout the city, counties or wherever needed (a reliable car, insurance and valid driver’s license is required for the job).
  • Knowledge of the code of ethics for social workers
