Overview
We are seeking a highly motivated, innovative, hard-working, and inclusive staff member to join our Aging and Adult Services Division as a Case Aide!
The Case Aide position supports the Options for Long Term Care (OLTC) Case Manager in implementing the service plan by communicating with internal and community resources and providing clerical support to case management/management staff.
OLTC provides access to publicly funded programs, providing support to people with disabling functional conditions and limited finances. These programs are designed to help people continue to live in their own homes or in other community settings, as an alternative to nursing home care.

Responsibilities
Specifically, this position:

• Completes administrative steps to assign clients to the appropriate Case Manager.
• Attends monthly unit meetings.
• Supports Case Management staff by logging and sending Prior Authorization Requests to service agencies
• Sends requests for paperwork from the unit staff.
• Distributes received mail and faxes to members of the unit.
• Coordinates Medicaid Home Modifications.
• Monitors client utilization of approved services by creating and tracking monthly reports to share with service agencies.
• Logs monthly budget statements into the internal database and communicates any budgetary issues with Case Managers.

Qualifications
All candidates will be screened for the following required minimum qualifications. Any candidate who does not meet all of the qualifications listed as required will be eliminated from the process.

• Possession of, or ability to obtain, a valid Driver's License is required.
• One (1) year of experience in a directly related field or in the performance of similar duties and responsibilities required.
• Bilingual (English/Spanish) skills preferred.
• High school diploma or GED required.

Supplemental Information
This position works Monday-Friday 8am-5pm and will be located at one of the following three locations: 1501 Blue Spruce Drive, Fort Collins, 2555 Midpoint Drive, Fort Collins, 200 Peridot Avenue, Loveland.
This is a hybrid position and will require at least one day/week in the office. You will be required to have adequate internet access and space for remote work. Larimer County provides an annual remote work stipend to assist with equipment and office supply needs for remote workspaces. This position requires a vaccine compliance badge.
All new employees and rehires will be required to submit fingerprints for a CBI criminal background check (c.r.s. 26-6-104).
An offer of employment is contingent upon the successful completion of a pre-employment drug screen and background check. As marijuana is an illegal substance under federal law, testing positive for marijuana (medical or otherwise) or any other substance for which the county tests, will result in the contingent offer of employment being revoked. Successful completion of the drug screen is defined as obtaining a negative result.
