Job Description

We are looking for compassionate and talented Care Coordinators (Case Managers) to be part of our exceptional, multidisciplinary team. As a Care Coordinator, you will play a crucial role in the community, bridging the gap between homeless individuals with serious mental illness, substance use disorders, and/or complex medical illnesses and the essential care they need. Our work is part of an innovative MassHealth initiative, providing qualified MassHealth enrollees with a service benefit called Behavioral Health Community Partners.
Your responsibilities will be diverse and fulfilling. You'll be part of an interdisciplinary BH-CP team, coordinating care for up to 50 enrollees. By engaging with them regularly, primarily face-to-face, you will ensure they receive vital services and make progress towards their goals outlined in their Person-Centered Care Plan.
Responsibilities
Communicate and engage with enrollees regularly to support their progress on identified goals.
Collaborate with nurse care managers to conduct timely Comprehensive Assessments and develop/implement Patient-Centered Care Plans.
Document enrollee progress, encounters, and care coordination notes in our data platform.
Assist with arranging appointments and accompany enrollees when needed.
Triage and troubleshoot care needs, advocating for enrollees' requirements.
Participate in team case conferences and BH-CP training sessions, continuously enhancing your skills.
Offer friendly reminders to enrollees about appointments and prescriptions.
Facilitate the exchange of medical records for care coordination purposes.
Qualifications And Skills
Experience in case management or human services is a plus but not required.
Bachelor's degree in social work, human services, nursing, psychology, sociology, or related field; or an Associate degree with relevant professional experience; or at least three years of relevant professional experience.
Excellent Customer Service Skills With Effective Communication Abilities.
Efficient, organized, detail-oriented, and capable of meeting time-sensitive tasks.
Self-directed, able to work both independently and as part of a supportive team.
Interest in using data to improve patient care.
Motivational Interviewing and Cultural Competency training are beneficial.
Experience with homeless populations and/or individuals with mental illness and substance use disorders is advantageous.
Proficiency in Microsoft Excel Spreadsheets and data entry; internet research skills.
