ORGANIZATIONAL SUMMARY:
The Department of Mental Health Law and Policy (MHLP) is one of six departments/schools in the College of Behavioral & Community Sciences and Department faculty are integral members of the Louis de la Parte Florida Mental Health Institute. The department promotes academic excellence by providing leadership and opportunities that enhance manpower development and personal growth in a fully engaged learner-centered environment. The mission of the department is to promote the health and quality of life of people with mental and substance use disorders by creating, evaluating, disseminating and translating state-of-the art knowledge to support effective practice across behavioral health and criminal justice systems.
POSITION SUMMARY:
The Pinellas County Coordinated Access Model (CAM) is a crisis hotline designed to help Pinellas County Residents access physical and behavioral health services and community resources. The Care Coordinator will be responsible for supporting and following up with residents during and after their referral process. The position will also assist the counselors to ensure the residents are connected to appropriate services and support.

This is a Staff Time- Limited position and it's 100% grant funded, continuation of this positon is based on funding. This position is not remote.
RESPONSIBILITIES:

  • Performs other duties as assigned.
  • Training: Complete all required initial training, including CAM Hotline onboarding, Mental Health First Aid for adults, Trauma Informed Care, Psychological First Aid and HIPAA training. Participate in all ongoing training, as required.
  • Care Coordination: Assist with coordination of referrals to community agencies for physical and behavioral health services. Follow up with residents after appointments to ensure all behavioral health and care coordination needs were met in a timely manner. This position will also follow up with providers to ensure residents showed up to schedule appointments and discuss any identified additional needs.
  • Self Management and Problem Solving: Ability to quickly assess problems and offer obtainable and creative solutions. Taking responsibility for one's behavior and well-being. Ability to work in a fast paced environment, requiring minimal supervision and oversight and able to work effectively under stress.
  • Program Communications: Relate to residents in an honest and sincere manner. All communications must be compassionate, clear, and solution focused.


POSITION QUALIFICATIONS:

Bachelor's degree in Social Work, Social Sciences, or related field.
PREFERRED:

  • Bilingual
  • Previous experience with behavioral healthcare coordination


SPECIAL SKILLS/TRAINING:

  • Ability to establish and maintain effective working relationships with other CAM staff and volunteers
  • Must be able to complete and pass a HIPAA training program
  • Knowledge of and compliance with the CAM policies and operating procedures
  • Intermediate skills in operating Windows-based computer operating programs
  • Excellent oral and written communication skills
