Key Responsibilities

• The ability to use independent judgement and discretion on matters of great significance to the company.
• Facilitates and delivers all aspects of the National Diabetes Prevention Program, including delivery through a distance learning format, content creation, reporting, and progress tracking.
• Collects information about a plan participant’s healthcare status and functioning to develop a comprehensive disease management plan for metabolic syndrome. Educates the plan participant regarding the disease process and expected treatment regime from clinical evidence-based standards in an effort to lower risk for development of diabetes.
• Conducts telephonic dietary consultations as requested for members participating in CCM and CM. Conducts follow-up calls as needed.
• Serves a subject matter expert on HealthComp Coach and identifies and creates content that can be used in conjunction with the NDPP and chronic care management program.
• Supports HealthComp’s wellness program through content creation and delivery of live and/or recorded webinars and content creation for nutrition-based challenges.

Skills, Knowledge & Expertise

• Must be bilingual in English and Spanish
• Must be computer literate and knowledgeable in Windows and Microsoft Office environment, including Word and Outlook. Microsoft Excel and knowledge of insurance database system a plus.
• High school diploma or general education degree (GED). Bachelor’s Degree, Registered Dietitian.
• Strong oral and written communication skills; Confidence in presenting and public speaking; Keen ability to multitask; ability to manage complex and unforeseen challenges.
