Behavioral Health Specialist/ Social Service Assistant

The qualified candidate for the Social Services Designee position must have a High School diploma as a minimum, with a degree in Social Work or related field preferred. 2 - 3 years social services experience in a long term care setting is a must. Experience dealing with mental health/behavioral population
Minimum Qualifications
Education/Training

• Master's degree (preferred), Bachelor’s Degree minimum in Psychology, Social Work, Occupational Therapy, Psychiatric Nursing or other mental health discipline, or an equivalent combination of education and related experience.

Experience
2-3 years experience in the assessment and disposition of behavioral health disorders preferred
Knowledge, Skills & Abilities

• Ability to provide therapeutic interventions, assessment, evaluation, and disposition. Well-established ability to make independent clinical decisions. Organizational skills. Interpersonal skills, including verbal and written communication. Basic computer skills preferred.

Primary Duties And Responsibilities

• Participates in the management of aggressive patients as needed to ensure the safety of patient, self, and others. Recognizes early warning signs of escalating behaviors and intervenes as appropriate.
• Serves as liaison to referral sources, outside agencies, third-party payors, and other interested parties as appropriate and within limits of confidentiality.
• Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations. Coordinates admission to programs and services within Behavioral Health Services and associated programs.
• Completes documentation relevant to activities. Complies with and ensures adherence to documentation standards. Complies with data collection, entry, and reporting requirements.
• Provides clinical assessment, evaluation, and disposition of persons presenting with behavioral health concerns or illness.
• Participates in multidisciplinary quality and service improvement teams.
• Performs other duties as assigned.
• Provides and coordinates referrals to internal or external resources as appropriate.
