About the Role

We are seeking an associate medical director to join our team full-time to serve both as an on-call clinician to grow our product and service offerings as well as advise in building the operations to support those offerings. You will be working clinically as a provider in conjunction with the Medical Director. Primarily, you will be on call for urgent care requests for an average of 50 hours a week depending on scheduling. You will also focus on developing clinical pathways that are evidence based and geared toward providing efficient and consistent care with a focus toward the elderly community.


You will be reporting to the VP of Clinical Operations, where your focus as an individual contributor will be to work collaboratively and cross-functionally to build effective products and services, and their respective operations, to support our vision of improving healthcare for America's seniors. We will be leaning on you to help define what we build and how best to operationalize the processes to deliver those products and services. Therefore, you'll drive value for Tembo Health and its customers through effective & impact-oriented decisions for our products and operations.


Responsibilities

Respond to urgent care requests. You will deliver exceptional, empathic, evidence-based virtual care to patients for common and acute care concerns in response to calls requesting urgent assistance and evaluation. This could include assessing chest or abdominal pain, injuries due to falls, extended fatigue/weakness, dizziness, etc. while abiding by all regulatory guidelines, as applicable to your license.

  • Develop clinical protocols and guidance. You will collaborate with the Medical Director to build evidence based protocols that are focused toward geriatric medicine. You’ll work with our teams to build products and services that effectively help the patient and enable clinicians to do their work. We want to build products for and by clinicians. Therefore, your valuable insight and feedback will help make sure we design with effectiveness, efficiency, and ease in mind.
  • Follow up on labs, images and reevaluations. Partnering with the clinical manager, you will perform follow up visits which will include reaching out to family members and patient’s PCP.
  • Ensure quality documentation and data. As an outcomes-focused company and since working with multiple stakeholders, you will ensure all charts and notes are filled out and medical records are completed using our EMR system and proprietary software.
  • Customer outreach. In order to create awareness and build relationships with our customers,, you will conduct friendly outreach calls to PCP’s, senior living facilities, patients, and families. Here, you will briefly describe our goals and purpose in these communities while also ensuring that we are not competing for the role of the patient’s PCP. You will communicate how we can work synergistically to improve the health and lives of residents.
  • Provide collaborative, clinical expertise. You’ll be working with other clinicians that may need your opinion and perspective on cases and patient treatment. When needed, you will provide support to colleagues with your experience. Additionally, we believe in shared decision-making where ultimately the patient, or their designated family member, has final authority on what to do. So we rely on you to provide as much relevant perspective for them to make an informed decision as one of their trusted healthcare partners.
  • Provide supervision of clinical staff. For some of our services like care or case management, you may need to provide supervision of our nurses and staff.
  • Drive process improvement. We will lean on you as part of the clinical operations team to think about how we can grow and scale what we do from a clinical perspective. You will have the opportunity to identify how to improve our clinical workflows and supporting operations. Work with different stakeholders to build the processes and workflows to make our services and team be successful.
  • Manage per diem clinicians. You’ll also help manage a team of part-time clinicians and their respective engagement, efficiency and efficacy. We will look to you to know how they are doing and for any opportunities to grow, support, and train them.
  • Conduct health assessments and intake visits. As new residents join a community or elect for our services, you will conduct a telehealth exam, review their medical history, and provide health risk assessments and screens for areas such as mental health/cognition or fall risk. This will all serve as helpful information and baselines for future visits while reducing friction and barriers to care even during emergency situations. We will also use this to determine qualification for programs like CCM and RPM.

Travel Requirements

We expect this role to potentially have up to 20% of travel for user research and stakeholder engagement at our customer communities.

Requirements

You’ll be a fit for this role if you:

  • You have a good bedside / "webside" manner and interpersonal skills when dealing with patients, physicians, and caregivers.
  • Board Certified in Family or Internal Medicine. Completed a residency in either Family Medicine or Internal Medicine with an active board certification and a license in good standing
  • Have an entrepreneurial, results-driven mindset with experience impacting efficiency, effectiveness, and engagement.
  • Effectively identify what’s important and prioritize competing workstreams in an ambiguous environment
  • Highly Comfortable with Digital Health technologies. You are adapt at telemedicine, and using an EHR (e.g., Athena Health).
