Job Description

The San Francisco Department of Disability and Aging Services have established the Temporary Respite Caregiver Support (TRCS) program to provide respite care to unpaid family members, family of choice, and/or individuals (e.g., friend) who are caregivers to older adults and adults with disabilities. The program can be utilized to provide respite through in-home caregiving or day program attendance. The role of the Assessment Specialist is to conduct the screening and assessment eligibility, connect the client to in-home care or day program services, and provide other caregiver support resources as needed.
Respite Care shall be provided only to a caregiver of a care receiver having two or more activities of daily living limitations or a cognitive impairment, residing in San Francisco County.
PRIMARY RESPONSIBILITIES:
Conducts assessments of caregivers who are in need of temporary in-home or out of home relief from caregiving responsibilities. Respite care is designed to reduce caregiver burden and intended to support quality home care that is instrumental in enabling the care receiver to live independently and prevent or delay the need for a higher level of care.
Maintains open communication and scheduled contact with caregiver and/or third parties to monitor ongoing respite support, information, referral, and assistance as needed.
Ensure a high level of customer service is provided and documented through a satisfaction survey administered by IOA and supported by DAS.
ESSENTIAL JOB FUNCTIONS:
  • Completes mandatory reporting to Adult Protective Services when necessary.
  • In collaboration with client and other supports, transitions clients out of the program when appropriate and documents the process as required.
  • Contributes to open lines of communication within the team. Attends and actively participates in team and program meetings, activities, and problem-solving endeavors. Participates in and promotes ongoing efforts towards continuous quality improvement.
  • Maintains required paperwork and follows a clear, concise, and consistent system of charting to allow for continuity of care. Documents all case coordination activity including monthly contact via electronic health record.
  • Evaluates respite care needs and follows up to determine if services have been provided in a timely manner. Listens to caregivers’ concerns and provides intervention or incorporates support networks as appropriate. Follows up to resolve interruptions and ensure ongoing respite support.
  • Understands and applies the regulatory and procedural requirements of the Temporary Respite Caregiver Support program.
  • Attends continuing education classes and/or in-service training to increase knowledge, skills and attitudes related to caregiver support, gerontology, family and community systems and other areas relevant to the TRCS client population.
  • Input’s assessment and demographic information (including age, race, ethnicity, gender, sex at birth, sexual orientation, and primary language) into electronic health record.
  • Utilizes supervision appropriately, maintaining open lines of communication and providing updates on caseload activity.
  • Other reasonably related tasks as assigned.
  • Conducts comprehensive in-person assessment.
  • Performs initial screening of potential clients via telephone and presents Client Orientation materials to educate clients about program policies, procedures, and respite vendors. Provides information and referrals regarding community resources when needed.
  • Based on a completed assessment, develops a client-centered service plan. Works with clients, support networks, and third parties to implement the service plan.
REQUIRED QUALIFICATIONS:
  • Bachelor’s Degree from an accredited school in social work, psychology, counseling, rehabilitation, gerontology, or a related field.
  • Experience with and understanding of the needs of unpaid caregivers supporting adults with disabilities and/or older adults.
  • Highly organized and detail oriented.
  • Exceptional communication, presentation, and problem-solving skills.
  • Ability to prioritize and manage multiple, varied, and fast-paced workflow.
  • One year experience working within a community-based programs and/or care systems.
  • Computer literacy.
DESIRED QUALIFICATIONS:
  • Demonstrated assessment skills.
  • Experience in a community-based care system.
Bilingual in Threshold Language(s) of Spanish, Cantonese, Mandarin, Tagalog, or Russian.
COMPENSATION:
Range: $ 27 - 32/hour
