Job Summary (Purpose and Function):

The Hematology/Oncology Nurse Practitioner Fellow’s role at the University of Miami, Sylvester Comprehensive Cancer Center is aimed at providing specialized training in the management of hematological and oncological patients in a variety of settings to ensure optimal quality of care and gaining necessary skill sets to develop and strengthen proficiencies within the clinical setting while learning to collaborate within a multidisciplinary team of healthcare providers.

The Nurse Practitioner Fellow will perform a patient assessment to include medical history and physical exam and in collaboration with the ARNP/physician mentor and develop an appropriate plan of care in the direct management of patients with acute and chronic illnesses.

The Nurse Practitioner Fellow works directly with staff, administration, patients and families to ensure quality care across the continuum as an expert in selected practice areas. The Nurse Practitioner Fellow integrates current knowledge of professional issues, trends in health care and technological advances into scope of practice. Additionally, the Nurse Practitioner Fellow participates in the collections of research data, quality improvement, discharge planning and coordination of patient care within the acute care areas.

The Hematology/Oncology Nurse Practitioner Fellowship program at the University of Miami, Sylvester Comprehensive Cancer Center will complete rotations in a variety of clinical setting located within in all UMHC/Sylvester sites to include both the inpatient and outpatient settings over a period of approximately 52 weeks.

The primary duties and responsibilities of this position include, but are not limited to:

Accountabilities:

I. Clinical Role

1. Provides care to a select group of patients by determining and rendering advanced nursing and medical interventions.

a. Assessment/Diagnosis: Assesses patients by interviewing/examining the patient, reviewing the patient’s medical history, physician’s diagnosis and orders, diagnostic test results (labs, x-ray, MRI, etc.) to determine medical diagnosis, plan of care and monitor patient’s progress.

i. Performs physical assessment.

ii. Reviews the medical record including physician/consultation notes, lab and diagnostic tests results. Formulates medical diagnoses based on above data.

iii. Collaborates with physician through discussion and assessment of diagnoses.

b. Planning/Outcomes: Develops a plan of care from the assessment/diagnoses and physician collaboration.

i. Identifies expected outcomes based on the assessment/diagnoses and physician collaboration.

ii. Includes interventions in plan of care that reflects expected outcomes.

iii. Documents and coordinates plan of care.

iv. Provides patient/family education regarding disease processes, treatment options, treatment outcomes, possible complications, medications, discharge planning and lifestyle modification.

v. Integrates current knowledge of professional issues, trends in health care and technological advances into scope of practice.

c. Implementation: Institutes necessary actions as outlines in the plan of care.

i. Collaborates with team members to ensure implementation of plan as described.

ii. Implements physician orders per protocols.

iii. Communicates plan of care to team members.

iv. Assists in the management of family dynamics and coping mechanisms during acute and chronic phases of patient care.

v. Engages in performance improvement activities to support the mental health needs of our patient population.

d. Evaluation: Evaluates the patient’s progress in attaining expected outcomes by follow up with collaborating physician.

i. Evaluates as warranted by patient’s condition, daily, by assessment of physical findings, lab and diagnostic test results.

ii. Formulates new plan of care as determined by evaluation.

e. Consultation/Collaboration: Consults with collaborating physician(s) on implementation, evaluation and revision of treatment plan.

i. Coordinates care with interdisciplinary health care team in the development and implementation of plan of care.

ii. Facilitates communication between the nursing staff and physicians with leadership skills and on-unit mentoring.

iii. Collaborates with members of the education department and units’ leadership team in planning educational opportunities and competency evaluation for nursing staff members.

iv. Conducts or participates in multidisciplinary rounds and ensures appropriate and quality care for patients.

v. Presents at multidisciplinary patient care conference.

vi. Rotates and refers to support services as needed.

II. Research

1. Initiates and/or participates in unit and/or hospital research activities.

2. Utilizes research finding and nursing theory in clinical practice.

3. Promotes evidence based/knowledge based nursing practice.

4. Collects data for patient and program evaluation and participates in quality assessment and improvement activities.

5. Authors or contributes to an article for publication in a professional journal or book.

III. Professional Development

1. Participates in 360 peer review process.

2. Submits annual protocols; revision to state nursing and medical boards.

3. Maintains national certification.

4. AOCNP certification upon completion of fellowship

5. The nurse fellow is expected to complete all competencies and training specified for all nurse practitioners working at UM/SCCC

6. Successful completion of ONS Chemotherapy & Biotherapy Course.

7. Attendance to one national conference.

8. Presentation of clinical case studies at completion of each clinical specialty rotation.

9. Attendance to weekly educational grand rounds.

10. Attendance to disease specific team meetings.

11. Attendance to educational sessions provided by department of nursing.

Supervision Received:

Reports to collaborating ARNP Mentor (assigned) during clinical rotations and collaborating physician. Reports directly to Director of Clinical Operations for Sylvester Comprehensive Cancer Center. The Fellow will also report to the Co-Leader of the Fellowship Program on a bi-weekly basis to evaluate the program experience and rotations.

Minimum Qualifications (Essential Requirements):

1. Graduate of Master in Science in Nursing with approved ARNP Curriculum or higher

2. Florida Licensure as ARNP from Florida Board of Nursing

3. Board Certification as ARNP from nationally accredited organization such as American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioner (AANP)

4. Current Certification in BLS and ACLS, Medical Errors (HIV/AIDS & Domestic Violence done on initial certification)

5. Excellent written & verbal communication skills

6. Good interpersonal relationship

7. Basic Computer knowledge in Microsoft

8. CITI certification within three months of employment
