Job Description
Description


Work in conjunction with referring physicians to help ensure appropriate admissions to the unit


Perform initial assessment with patient and family


Assist with the development and implementation of initial Plan of Care


Ensure continuity of care throughout the admissions process


Ensure documentation is in accordance with established standards and procedures


Assist in the review/revision of policies and procedure related to the admissions process


Attend interdisciplinary team meetings


Participate in Quality Assessment and Performance Improvement activities


Provide staff education on the admissions process and any changes that are implemented


Responsibilities


Current GA RN license or license eligible


BSN preferred


Nursing Experience Preferred
