Position Type/Expected Hours of Work: 40 hours, flexible to the needs of the program. Hours are divided between the Day Habilitation and Residential Sites.
Qualifications: Bachelor’s Degree in Human Services or related field and one (1) year experience working with the developmentally disabled; or Associates Degree in Human Services or related field and two (2) years’ experience working with developmentally disabled individuals; or High School Diploma or equivalency with five (5) years’ experience working with developmentally disabled individuals. Experience in recreation, fitness and/or activity planning preferred. Must have valid/acceptable NYS driver’s license.
Work Authorization/Security Clearance: Authorized to work in the United States and pass all background clearances.
Summary/Objective: Assist in coordination of activities for the People We Support, stressing independence, inclusion, individualization and productivity.
Essential Functions: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
- Serve as Advisory Board Facilitator.
- Coordinate, facilitate, and participate with wellness and recreational activities for the People We Support in the day and residential programs, including community and agency sponsored events that engage community members.
- Establish a monthly schedule of activities and events for integration in the community. Activities could include but are not limited to: clubs, gym activities, pool, arts and crafts, music, community events, etc.
- Adhere to the guiding principles of the Agency Corporate Compliance plan in Section 303 of manual.
- Attend and satisfactorily complete all required in-services.
- Provide ideas, direction and input for activities for People We Support. This should include obtaining input from the People We Support in the selection of the activities.
- Evaluate needed recreational supplies and equipment for sites and coordinate the ordering of such, following all approval processes.
- Ensure adequate transportation is provided to events to include coordinating with the transportation department.
- Follow all program plans as indicated and provide documentation for any goals run through recreational activities. This will involve familiarity with Therap.
- Submit monthly schedules to interim Assistant Director of Habilitative and residential Services on the third Friday of each month.
- Complete incident/accident reports on an as needed basis.
- Facilitate relationships with community organizations and businesses to increase recreational opportunities for People We Support, ensuring activities take place in natural environments.
- Assist the support teams in addressing Personal Outcome Measures by arranging for recreational and community activities that the People We Support wish to engage in.
- Work with staff at day and residential sites to facilitate the involvement of the People We Support in recreational and fitness activities both in the community and at the agency.

Travel: Travel is routinely expected throughout Jefferson and St. Lawrence counties or other areas as deemed appropriate for the needs of the people we support.
Work Environment: This job operates out of the homes of the people we support, the site based program building, and in the community where people we support receive program services. This role routinely use standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; lift; use hands to finger, handle or feel; and reach with hands and arms. Work may require lifting, pushing, or pulling in excess of 30 pounds.
