Bachelor’s degree in Social Work or a closely related field with 8 years of relevant, direct practice experience.

Clinical skills (no license required) and crisis intervention skills.

Master’s degree in Social Work or a closely related field with 4 years of relevant. direct practice experience

Previous experience working with youth and with the Tennessee Department of Children’s Services is preferred.

Applicants should be proficient in developing and delivering presentations.

Proficiency in Microsoft Word and PowerPoint is also required.

Directly supervise interns in their fieldwork, overseeing the entire range of field experiences including ensuring activities are meeting field education objectives.

Provides direct service and consultation during threat assessment/management, crisis response, recovery and renewal.

Provide management with statistical and project/program progress reports.

Knowledge of the policies and state laws that surround education and the underserved populations

Knowledge of evidence-informed culturally appropriate interventions for children, youth, and families that can best help them to achieve educational goals

Ability to work with various levels of employees and management

2+ years previous experience working in care management and/or with chronic illness within a medical environment i.e. home health, dialysis, hospice

Coordinate and referee games as needed

Certification in Case Management strongly desired

Experience working and providing collaborative care management interventions with various State Agencies such as DMH, DDS, DCF, DYS required.

Works with members of the Utilization Management Department assisting with difficult or complex care delivery or discharge planning needs for members

Previous experience working at a Managed Care Organization preferred.

Performs other responsibilities as assigned by a member of the Clinical Integration Leadership Team

Participates in monthly supervision and one on one meetings with the manager
