Conducts case management activities by assisting the licensed clinicians with the assessment process.

Assist clients in completing initial paperwork necessary for evaluation.

Interviews clients to assess the need for referrals to outside agencies and completes referrals to the proper sources when necessary.

Assist in getting community resources and taking Medicaid applications.

Assists staff with the completion of consumer care requests.

Obtain information and records on clients regarding previous behavioral health treatment.

A maximum of 120 semester hours may be combined with experience to substitute for the baccalaureate degree.

Requirements For a Social Work Counselor

Knowledge of interviewing skills and techniques

Collaborates with client manager and assists in developing wrap-around transition and aftercare services for the youth nearing parole.

Collaborates with educational/vocational systems to ensure appropriate academic services are provided.

Position is responsible for maintaining all professional certifications.

Actively participates in own professional growth and continued trainings, research, and Continuing Education Unit’s.

Counseling will be provided in accordance with training and expertise gained through specific training and knowledge of DORA standards and ethics.

One year of experience as a Licensed Clinical Social Worker or Licensed Professional Counselor.

One year Experience as a licensed marriage and family therapist will substitute for the required experience on a year-for-year basis.

Work experience must be specifically documented on your application.

Determines appropriate, strengths-based treatment goals and objectives for youth.

Determines appropriate individuals to provide treatment and other servicec to youth per treatment plan.
