KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED BY THE POSITION

Works with clients to identify health, social, economic, and educational needs, by conducting client assessments and ongoing interactions with the clients.

Network with community-based organizations to identify available resources and develop partnerships to benefit clients.

Other (e.g., post-offer medical exam, polygraph, background check, driver’s license

Educate clients about treatment programs, public assistance programs, and other resources to improve access to care and support recovery.

Excellent verbal and written communication skills - bilingual Spanish/English preferred

1+ years of Customer Service experience on an app based platform is preferred

1+ years customer Service experience in healthcare is also preferred

Basic Life Support (BLS) or equivalent certification within first three (3) months from Date of Hire, required.

Demonstrated knowledge of patient transfer/transport techniques, and related body mechanics.

Knowledge of basic related Safety and Infection Control policies and practices.

Receives formal instruction/direction upon pick-up and provides formal hand-off report at destination, when not required to stay with patient.

Reports any changes in patient status/condition during transport.

Removes and reports defective equipment to supervisor.

One (1) year of case management experience

Maintains precise and accurate documentation of case management services, including client files and entries into the program databases.

Two (2) years of experience in lieu of education

Performs other duties as assigned.

OTHER KNOWLEDGE, SKILLS, AND ABILITIES:

Bachelor's degree in a social services-related field, mental health, elementary/secondary education, prevention or equivalent experience

Must demonstrate a high degree of organization and efficient time management

Experience in addressing the needs at-risk populations, including those experiencing homelessness

Knowledge of Upper Peninsula agencies and resources to support students and their families

Assist local education agencies (LEAs) meet the needs of students experiencing homelessness and decrease
