Senior Care Managers are responsible for assessing the needs of seniors and their families, developing and implementing care plans, and monitoring the progress of seniors. Detailed Job Description Senior Care Managers are responsible for providing comprehensive care management services to seniors and their families. Senior Care Managers coordinate and manage the care of seniors in a variety of settings, including nursing homes, assisted living facilities, and home health care.
