5 years of experience as a Registered Nurse

2 years of experience in home health nursing

1 year of experience supervising home health cases or home health clinicians

Previous experience using HCHB, both R2 and PointCare is highly preferred.

Ensure that patient care and services are delivered appropriately and timely

Impact patient outcomes positively, reduce unnecessary hospitalizations and trips to emergency departments
