They also provide support and guidance to clients and their families. Detailed Job Description A Remote Case Manager is responsible for providing case management services to clients. They must assess the client’s needs and develop a plan of care that meets those needs. They must coordinate services with other providers, monitor the client’s progress, and evaluate the effectiveness of the services provided. They must also provide support and guidance to the client and their family. They must be able to work independently and be able to communicate effectively with clients and other providers.
