Operations, leadership, critical thinking, analysis, teaching, facilitation, coaching, mentoring and project management skills required

Knowledge of population health management practices, policy, programs, initiatives and functions.

Course work in Quality Management, Regulatory Compliance, and Statistics preferred.

Knowledge of and experience in process improvement (e.g., lean or process transformation).

8 years recent relevant experience

Exemplary written and oral communication skills; to include presentations and comprehensive analysis reporting on performance improvement issues.

The Patient Flow Center Coordinator will also serve in the role of Bed Management System (BMS) administration.

BSN or higher, Minimum 1-2 years working in utilization management or flow coordination

Previous medical surgical nursing experience

American Red Cross or American Heart Association Basic Life Support Course (BLS or BCLS) and Certification

American Red Cross or American Heart Association Advanced Life Support Course (ALS or ACLS) and Certification within 90-days of hire

American Red Cross or American Heart Association Pediatric Advanced Life Support (PALS) Course and Certification within 90-days of hire

Three (3) or more years hospital/acute care experience. Emergency medicine experience preferred

Proven clinical & leadership experience preferred.

Associates Degree required (if enrolled in BSN program)

2- 3 years Safe Patient Handling and Movement experience

5 or more years of experience in reviewing medical records.

Strong collaboration skills with key stakeholders, both internally and externally.

Experience in working with the entire Microsoft Office Suite to include Word, Excel, PowerPoint, and Outlook.

Experience with clinical quality, quality improvement, and/or risk management;

Detail oriented and highly organized with effective time management skills.

Include all quality and delivery requirements, including any certifications that may be required.

2 years of Behavioral Health experience

2 years of Med Surge experience

Clinical quality experience is a plus

Prior supervisor and quality management experience.

Duties and responsibilities include, but are not limited to, the following:

Degree in nursing RN (Required) with at least 2 years experience

Experience working with chronically ill (CKD/ESRD) patients

80% On location Travel for Patient Home Visits and (Practice Office)

Enhancing a collaborative relationship to maximize the patient’s/family’s ability to make informed decisions

Participating with Care Team and physicians to prevent readmission, prevent future hospitalization and improve patient care

Conducts tracers, rounds, and investigations across the organization and communicates opportunities to local management and leadership as appropriate.

Advises staff and management of patient safety concerns and evaluates situations for risk to patient safety.

Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
