As a Home Health Quality Coordinator you need to know how to:

Perform quality assessment of clinical records including diagnosis coding, review of assessment and plans of care.

Collaborate with leadership to develop quality indicators and QAPI initiatives for agency.

Team up with associates to ensure that services are provided at quality levels consistent with professional standards and goals.

If you are relying on your education to meet qualification requirements:

Functions case manager and liaison for planning, directing, outreaching, and coordinating care for women Veterans

Triages telephone calls, secure messaging, and walk-in patients and offers same-day appointments

Tracks high-risk pregnant patients throughout pregnancy and provides triage and education

Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).

Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.

1-3 years Three years clinical experience in a hospital, acute care, home health/hospice, direct care or case management required. Required

Knowledge and understanding of case management/coordination of care principles, programs, and processes in either a hospital or outpatient healthcare environment.

Prefer experience with Medical Management platforms used to document care coordination services.

Problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action.

Experience with windows-based computer programs and ability to use computer for data analysis and data display required.

RN-Registered Nurse - DORA - Department of Regulatory Agencies Required

Expand knowledge and capabilities by attending educational workshops, conferences etc.

Must has knowledge of Medicare/Medicaid and insurance reimbursement procedures, contracts, clinical resource utilization and/or case management.

Knowledgeable of nursing and medical practices and procedures as well as regulations and guidelines that pertain to skilled nursing
