Job Description

MDS Coordinator, Registered Nurse- (CASTLE ROCK, COLORADO)

Brookside Rehabilitation and Wellness; a sub-acute rehabilitation-short stay facility, is looking for a full-time RN MDS Coordinator. The ideal candidate will be a responsible and well-trained individual looking to ensure excellent survey results, staff excellence, clinical excellence, customer excellence, and business excellence.

Rate of Pay

$72,000-$85,000 annually depending on experience

Responsibilities

  • Maintain strong working knowledge of the Resident Assessment Instrument (RAI)
  • Review CMS Reports to identify assessments completed or submitted late and develop systems and processes to prevent reoccurrence
  • Maintain or obtain RAC-CT credentialing through the American Associated of Nurse Assessment Coordinators
  • Assist in the evacuation of patients when necessary.
  • Review Final Validation Reports and attest that all assessments have been completed and accepted into the CMS QIES system prior to billing and notify Business Office when assessments are not ready to bill
  • Report any changes in patient’s condition identified by the MDS Assessment to the Director of Nursing
  • Coordinate and assist with completion and submission of accurate and timely interdisciplinary MDS Assessments, CCA’s, and Care Plans according to CMS RAI Manual Regulations and in accordance with all applicable laws, regulations, and Brookside Rehabilitation and Wellness standards.
  • Perform functions of a staff nurse as required.
  • Attend and participate in the Daily PPS Meeting, Monthly Triple Check, and other meetings upon request.
  • Provide education to direct care associates regarding updates or changes to the CMS RAI Manual or Skilled Nursing Facility Regulations that impact documentation
  • Ability to assist with set-up and review of the Interdisciplinary Comprehensive Care Plan
  • Expand knowledge and capabilities by attending educational workshops, conferences etc.
  • Must be able to sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours.
  • As part of the Nursing Leadership Team, this position will also have management and leadership expectations as part of their role in the Community including on-call duties per rotation schedule.
  • Foster a supportive and compassionate environment to care for patients and their families

Skills

  • Must perform proficiently in all competency areas including but not limited to: general MDS responsibilities, patient care, patient rights, and safety and infection control
  • Ability to communicate policies, procedures, regulations, reports, etc., to personnel, residents, family members, visitors, and government agencies/personnel
  • Must has knowledge of Medicare/Medicaid and insurance reimbursement procedures, contracts, clinical resource utilization and/or case management.
  • Must be able to read, write, speak, and understand the English language
  • Ability to assess residents and relate information regarding resident’s condition.
  • Responsible and compassionate
  • Must have good interpersonal communication skills, both verbal and written.
  • Ability to make independent decisions when circumstances warrant such action
  • Knowledgeable of nursing and medical practices and procedures as well as regulations and guidelines that pertain to skilled nursing
  • Basic computer and typing skills
  • Must be able to sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours.
  • Ability to communicate and function productively on an interdisciplinary team.
  • Must be familiar with MDS Coding
  • Outstanding organizational and multi-tasking skills
  • Current, unencumbered, and active license to practice as a Registered Nurse in the state of Colorado
  • Patient with excellent problem-solving skills
  • Demonstrate dependable, regular attendance

Experience:

  • Graduate from an accredited school of nursing
  • EMR systems, 1 year (Preferred)
