Manager Of Care Management Jobs

Provide oversight to the care management and transition of care programs using resources appropriately to improve care and member outcomes.

Provides oversight and management of the CMS Model of Care and performance metrics.

Provide leadership to manage administrative healthcare costs and quality targets by using metrics to drive constant process improvement.

Manage the clinical assessment division to ensure UAS Assessments and quality reviews are conducted in accordance with regulations.

3 years of job related experience in a leadership role.

Use of internal and external benchmarks to drive high quality outcomes and efficient processes.

3+ years of experience managing, coaching or developing staff; 2+ years of program or clinical operations management

Working knowledge of case management systems, community service and delivery systems, and proficiency with Microsoft Office suite.

An active RN license is strongly preferred. Additional state-specific qualifications may be required.

Plan, organize, and oversee timely completion of activities, including assessments, care plans, and discharge planning

Implement and interpret organizational and contractual policies and procedures; regularly review work to ensure policies and guidelines are appropriately applied

Perform other duties and special projects as assigned

Experience leading care management teams. (R)

Solid problem solving and time management skills

Serve as a clinical resource; attend Care Management rounds.

5+ years of Clinical experience

Excellent communication skills (written and verbal)

Train employees in process operations; act as a preceptor to newly hired staff

Excellent leadership and team management skills

Minimum of five (5) years of previous experience in social services and discharge planning within an acute care hospital setting

Strong technical knowledge of social work and discharge planning processes

Bachelor's Degree in a relevant field

Valid license and current registration issued by the New York State Board of Nursing

3 years prior management experience

Experience in Care Management, Home Care and or Independent Practice Associations

Certification in Case Management preferred

Conducts, regular random review of cases on a weekly basis for care management interventions, outcomes, collaboration

Identifies areas for improvement of existing operational policies and procedures and develops recommendations to the Director of Care Management

Interacts with the Medical Director or designee for care management activities
