Conduct health related educational presentations to both adults and teens in school and clinical settings.

ESSENTIAL DUTIES AND RESPONSIBILITIES (

Responsible for providing therapeutic services by interviewing families, individuals and groups.

Assist in the assessment, planning, arranging, and implementing of psychosocial programs.

Counsel patients and interpret to professional and lay groups the special psychosocial needs of patients by identifying psychosocial disorders.

Building strong rapport with clients and making accurate assessments of their problems whether mental/emotional or physical.

Participates in interdisciplinary team meetings and utilization management rounds and provides information to assist with safe transitions of care.

Preferred experience with substance abuse population.

Experience with adults and adolescents.

Collaborates with member, family and healthcare providers to develop an individualized plan of care.

Conducts individual counseling and group therapy with adolescents and adults.

Identifies and initiates referrals for social service programs – including financial, psycho-social, community and state supportive services.

Assists patient-inmates/youths in applying for entitlement benefits (SSI), community programs, and services; Facilitates patient-inmates/youth's transition between various settings within the institution.

1 year of LCSW experience.

Consults on complex cases with supervisor, and treatment team members.

Documents all care provided to the inmate, including face-to-face contacts and Interdisciplinary Treatment Team meetings.

Monitors inmate progress using approved documentation formatting for progress notes.

Provides written reports and consults with the Board of Prison Hearings.

Three to five years of post-master’s experience working with children, adolescents, families and adults

Good listening skills, alertness, problem solving skills under stressful situations.

Performs initial assessments, evaluations, diagnoses, treatment, follow up, and education relevant to the clients care (age specific).

Assists individual students in addressing obstacles that interfere with personal/social, career or educational development.

Master’s Degree from an accredited school

Current licensure in good standing in the state of Louisiana

Some Of The Responsibilities Are As Follows

Review ED patients for psychosocial needs, readmission risk, or complex discharge needs

Respond to trauma, stroke, cardiac arrests, and other ES1 level 1 patients

Assess ED <3-day revisits and document findings and interventions in Cerner

Initiate and coordinate discharge planning on ED patients not being admitted to the hospital

Assess and intervene with families
