2 LCSW needed for evening shift (3-11pm)

Looking to hire three Licensed Social Workers to work in the Emergency Room (ED). Candidates should have current/previous ED experience and familiar with discharge planning, gatekeeping, and being able to case find. This is a fast paced and high-volume environment and will involve working with high-risk patients with ambulatory functions.

Some Of The Responsibilities Are As Follows

• Initiate and coordinate discharge planning on ED patients not being admitted to the hospital
• Assess ED <3-day revisits and document findings and interventions in Cerner
• Review ED patients for psychosocial needs, readmission risk, or complex discharge needs
• Respond to trauma, stroke, cardiac arrests, and other ES1 level 1 patients
• Assess and intervene with families
• Escalate needs as appropriate
• Conduct psychosocial assessment and intervention, integrate into plan of care (grief & bereavement, crisis intervention, trauma/alias identification, abuse/neglect, adjustment to illness/new diagnosis, miscarriage, advance directives/OOH DNRs, guardianship and/or protective services for patients with significant mental status impairment or unsafe living environment).

• Collaborate with ED Utilization Review (ED UR RN) for complex patient problem resolution or when coordination of significant or intensive community resources is necessary
