Attend staff meetings, team conferences and educational in-services, per agency requirements

The OPCM RN utilizes advanced clinical judgement and critical thinking skills to perform care management duties.

This role manages a caseload of between fifty (50) and seventy-five (75) members enrolled complex care management.

The Skills And Experience You Bring:

Activities include clinical assessment, member/family/caregiver education, development, and implementation of individualized care plans.

The OPCM RN works collaboratively with BTP inpatient care managers, Referral Services, Population Health, and other internal teams.

The OPCM RN engages with BTP primary care providers, and specialists, as well as hospital, skilled nursing, and health plan partners.
