RN Case Manager ($75k - $95k/year)

Ensures all care delivered is within the scope and established standards of safe professional nursing practice and HSL core competencies

Adheres to best practice recommendations and implements evidence bias evidence-based to ensure optimum outcomes for patients

Practices in a primary nursing model and maintains accountability for a set of assigned patients

Graduate of an approved school of nursing required; BSN preferred

Assesses, plans, and evaluates nursing care to culturally diverse adult/geriatric veteran populations

Overseeing nursing staff and veteran care activities on assigned units.
