Graduate from an accredited school of Nursing as a Licensed Practical Nurse require

Current Georgia License as a Practical Nurse required

Current American Heart Association Basic Life Support Health Care Provider Card required.

This position is located at Grady's

Two years minimum experience in an Outreach setting, Managed Care, Case Management, Ambulatory clinic setting, or related field.

Collaborating with Care Management Team if preventative health care coordination efforts are unsuccessful.

Ability to move through multiple management systems simultaneously.

Other duties as assigned to assist in the Care Management of health plan members.

Respond to incoming calls to the Care Manager with the goal of one call resolution or resolution within the same day.

Gather necessary information if the call needs escalation to Care Manager or supervisor.

Take vital signs and performs all ancillary tests appropriate to skill level, certifications and state regulations

Provides after-hours consultation for case management of employees’ medical care as a result of injury as needed

Maintaining the appropriate continuing education so that licensure/certification is ongoing

Working knowledge of occupational medicine requirements

Completes required certifications; becomes proficient in clinical systems and procedures, and maintains currency in all policies, practices, procedures and HIPAA training

Minimum of 1-year of directly applicable work experience

Previous experience in nursing or other medical fields

Excellent written and verbal communication skills

Care for ill, injured, or convalescing patients

Provide basic patient care and treatment

Collaborate with registered nurses to administer prescribed medications

Sterilize and prepare medical tools and equipment

Minimum Education and Experience Requirements:

One year of clinical-based experience. Specialty of home health preferred.

Successfully completed 30 hours of IV therapy course.

Demonstrates willingness and ability to maintain clinical competency as required by assigned unit.

Employees in this position will operate vehicles for an assigned business purpose as a "frequent driver."

Typically requires Bachelor’s degree or equivalent and less than 3 years of technical experience

Utilize a systematic, individualized goal/outcome driven approach in implementing the nursing plan of care.

Prepare, submit and maintain documentation as required by the Company and/or facility. Visit/shift notes documented on day services are rendered.

Provide education to patients and families on proper home health care procedures and strategies.

Must have at least 6 months minimum experience, Have a valid Texas Nursing License and willing to travel.

Assess and chart observations of the patient’s condition at each visit.

Complete evaluation tasks, including reviewing medication and vital signs.

Dress or redress wounds and assess the healing progress.

Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
